Paid holidays: 11 days. Vacations with pay: 2 weeks after 1 year, 3 weeks after 5 years, 4 weeks after 10 years, 5 weeks after 18 years for em- ployees hired after Feb. 26, 2000. 2 weeks after 1 year, 3 weeks after 3 years, 4 weeks after 8 years, 5 weeks after 13 years, 6 weeks after 18 years, 7 weeks after 23 years for em- ployees hired before Feb. 26, 2000. Overtime: Time and one-half for all work after 7.5 hours per day, 37.5 hours per week. Time and one-half when fourth or fifth day of holiday week is worked. Medical benefits: Employer pays 75% of premiums, em- ployee pays 25% of premiums. Employer will provide $200 per year health spending ac- count for part-time employees who have completed 1 year of service. Dental: Employer pays 75% of premiums, employee pays 25% of premiums. Coverage of $1,500 combined basic/ major maximum per year for basic services (recall exam ev- ery 9 months); major services (crowns, bridges, dentures); 50% for orthodontics for dependent children younger than 17, life- time maximum of $1,500. Vision: Employer pays 75% of premiums, employee pays 25% of premiums. Maximum $225 per person every 2 years Weekly indemnity: Employer pays 75% of premiums, em- ployee pays 25% of premiums. 66 2/3% of employee's average straight time (not including pre- miums) weekly earnings in 2 pay periods immediately preceding date of benefit eligibility, maxi- mum $600 per week. Payable on and from fourth day of illness, first day of accident, minimum 26 weeks of any 1 illness or ac- cident. Sick leave: One-half day for each month of employment, maximum 6 days; one-half day for each month of employment, maximum 4 days for employees hired after Oct. 15, 2014. LTD: 60% of employee's average straight time (not including pre- miums) weekly earnings in 2 pay periods immediately preceding date of benefit eligibility, maxi- mum $2,000 per month. AD&D: Employer pays 75% of premiums, employee pays 25% of premiums for coverage of $40,000. Life insurance: Employer pays 75% of premiums, employee pays 25% of premiums for cov- erage of $40,000. Pension: Employer will contri- bution $15.82 per day to fund, maximum $79.08 per week, per employee to BCTGM Union and Industry Canadian Pension Plan. Bereavement leave: 3 days for death of family member (deter- mined as per Employment Stan- dards Code, RSA 2000, c E-9). Seniority – recall rights: 12 months for layoffs. Probationary period: 45 work- ing days. Discipline: Sunset clause is 24 months for verbal or written warnings. Severance: 1 week of pay to start; 1 week for each year of service, maximum 30 weeks after 2 years of service if amal- gamation; permanent closure of plant or department; automa- tion causing full-time employee to lose employment.
