scheduled to work on holiday, not including Christmas Day, Boxing Day, New Year's Day. Vacations with pay: 1.25 days for each completed month of service to start, 3 weeks after 1 year, 4 weeks after 3 years, 5 weeks after 11 years, 6 weeks after 20 years, 7 weeks after 25 years. Full-time nurse teachers receive 1 additional week of paid vacation at either spring break or Christmas break. Overtime: Time and one-half for work in excess of 7.5 hours in 1 day or 37.5 hours per week averaged over 2 weeks. Time and one-half for work on paid holiday. Meal allowance: $6 or hot meal for nurses who work second consecutive full tour or more than 2 hours of over- time. Medical benefits: Employer pays 100% of premiums for Ontario Health Insurance Plan and Liberty Health semi- private plan. Dental: Employer pays 75% of premiums, employee pays 25% of premiums for Lib- erty Health Dental #9 plan or comparable coverage for regular recall exams; 50/50 up to $1,000 for dentures; 50/50 coverage up to $2,000 for crowns, bridgework, implants and repairs; 50/50 coverage up to $2,000 lifetime for or- thodontics. Vision: Employer pays 75% of premiums, employee pays 25% of premiums for cover- age of $450 every 24 months including laser surgery and 1 eye exam per insured person. Paramedical: Employer pays 75% of premiums, employee pays 25% of premiums for Liberty Health extended health care benefits plan pro- viding for $22.50 single and $35 family deductible. Chi- ropractic, massage therapy, physiotherapy up to $400 per insured person annually. Mental health services up to $800 per insured person per year. STD: Employer pays 100% of short-term sick leave plan at least equivalent to 1992 HOODIP. Maximum 562.5 hours. LTD: Employer pays 75% of premiums, employee pays 25% of premiums for long- term disability portion of sick leave plan (HOODIP or equivalent). Payable after 17 weeks of absence due to ill- ness at 65% of regular earn- ings, up to week 30. AD&D: Employer pays 100% of premiums for coverage equal to amount of group life insurance. Life insurance: Employer pays 100% of premiums un- der HOOGLIP or such other group life insurance plan cur- rently in effect. Employees also have access to voluntary life insurance plan (HOOV- LIP). Bereavement leave: 4 paid working days for death of parent, sibling, spouse, child, child-in-law, parent-in-law, sibling-in-law, grandparent, grandchild, spouse's grand- parent. 1 paid day to attend funeral of aunt, uncle, niece, nephew. Seniority – recall rights: 36 months; 1 year for transfer outside bargaining unit; 18 months for pregnancy or pa- rental leave. Call-in pay: Double-time for all hours worked with mini- mum of 4 hours' pay. Probationary period: 70 tours (525 hours for nurses whose regular hours of work are other than standards workday). May be extended to maximum of 60 additional tours (450 hours). Nurses who transfer from casual or regu- lar part-time to full-time sta- tus don't need to serve proba- tionary period if they already have since date of last hire. Uniforms/clothing: Nurses working in speciality units/ar- eas where scrub gowns or lab coats are provided at no cost, will continue to have such service provided by hospital at no cost. Such practice will not be discontinued without 60 days' notice to union. Mileage: Greater of $0.22 per kilometre or hospital policy when nurse is required to travel to hospital or return home as result of reporting to work between midnight and 6 a.m. or at any time while on standby. Longer daily tour may be discontinued in any unit when 51% of nurses indicate by secret ballot or hospital finds adverse effects on patient care or is unable to provide workable staffing schedule. Likewise for 10-hour tours. Nurses who indicate they don't wish to work 10-hours will be scheduled to normal 8-hour tours where possible. Hearing aid: $700 per person every 36 months. Standby: $3.45 per hour for period of standby scheduled by employer; $5.05 per hour when standby duty falls on paid holiday.
