Employer pays 100% of premi- ums for employee assistance program. Employer will provide $475 per year for health spend- ing account. Dental: Employer pays 80% of premiums, employee pays 20% of premiums. Vision: Employer pays 80% of premiums, employee pays 20% of premiums. Sick leave: 1 day per month (previously 1.5 days per month), maximum 60 days. STD: Benefits are mandatory after completion of probation- ary period. Employer pays 80%
