May be banked, maximum 45 straight-time hours. Time and one-half for first 2 hours after normal daily shift or after normal weekly shift. Double time after 2 hours of overtime or on scheduled day off. Medical benefits: Employer pays 100% of premiums for Medical Services Plan of B.C. and for Pacific Blue Cross Plan. Dental: Employer pays 100% of premiums for basic plan, 60% of premiums for extend plans B and C. $2,750 lifetime max- imum for orthodontic services. Vision: $350 every 24 months per employee or dependant. Sick leave: 1.5 days for each month of service, maximum 156 days. Employees leaving workforce on or after 55th birthday will be entitled to cash payment equal to 40% of value of accumulated credits. LTD: Employer pays 100% of premiums. AD&D: Employer pays 100% of premiums. Life insurance: Employer pays 100% of premiums for coverage of $50,000. Pension: Municipal Pension Plan. Bereavement leave: 3 days for death of spouse (including common-law), child, stepchild, parent, stepparent, brother, sister, stepbrother, stepsister, mother-in-law, father-in-law, grandparents, grandchild, legal guardian, relative residing in employee's household. 2 addi- tional days for travelling time when warranted in judgment of employer. Call-in pay: Minimum of 2 hours' pay at appropriate overtime rates for each separate call-in. May be paid out in cash or taken as time off. Employee called in to worksite, office or client's home will receive travel allowance of mileage allowance at rates set by Canada Revenue Agency or if public or private transportation facilities are not available, taxi fare from home to hospital and return, minimum $2 for each round-trip. Probationary period: 3 months of employment for probationary employees; 1 year for post-pro- bationary employees. Discipline: Sunset clause is 18 months for disciplinary actions, including letters of ex- pectation; 18 months following expiry of suspension for records of suspensions. Uniforms/clothing: Employer will provide uniforms to em- ployees that will be laundered by employer. Medical benefits: Employee pays $90 per month for health and welfare benefits, after 60 days of service. Dental: 90% reimbursement for basic services. 50% reimburse- ment for major restorative ser- vices after deductible of $25 per individual (maximum $50 per family), maximum $1,000 per individual per year. Sick leave: 1.5 days for each month of service, maximum 156 days.
