COLLECTIVE AGREEMENTS COLLECTIVE AGREEMENTS pays 100% of premiums. Dental: Employer will con- tribute $0.43 per hour to United Food and Commercial Workers' Dental Care Plan. Vision: Employer pays 100% of premiums. Coverage of $200 per person every 24 months for purchase, repair of prescription lens, frames. Reimbursement for eye exam (maximum $60) and for cor- rective lenses will be com- bined maximum of $200 (with no deductible) in 24-month period. Weekly indemnity: Employer pays 100% of premiums. Cov- erage of 70% of straight-time weekly wages. Sick leave: 4 hours for each month of employment, maxi- mum 208 hours. LTD: Coverage of 50% of base weekly earnings at date of dis- ability, maximum $2,000 per month (previously $1,800 per month). Life insurance: Employer pays 100% of premiums. Pension: Employee will con- tribute $0.22 per hour after 2 years; $0.40 per hour after 8 years of service, employer will contribute $1.45 per hour to Canadian Commer- cial Workers' Industry Pen- sion Plan. Bereavement leave: Leave of absence with pay, with con- sideration given to travel time to attend funeral, for death in immediate family (spouse, parent, stepparent, child, brother, sister, mother-in-law, father-in-law, sister-in-law, brother-in-law, son-in-law, daughter-in-law, grand- mother, grandmother-in-law, grandfather, grandfather- in-law, grandchildren, any relative living in household of employee). 1 day for death of aunt, uncle. Call-in pay: Minimum 4 hours of pay at regular hourly rate. Probationary period: First 200 hours worked. Discipline: Sunset clause is 24 months for discipline no- tices. Uniforms/clothing: Em- ployer will furnish smock or apron and employer will pay for repair and launder- ing of same for employees in bakery production and meat cutters in Lloydminster. In other departments where employer requires employee to wear smocks or aprons, employer will provide and repair smocks and aprons free of cost. Special cloth- ing — such as rain capes and parkas — will be supplied by employer. Gloves will be made available for use in cold weather. Employer will supply rubber boots to employees in produce and meats (Lloyd- minster). Employer will sup- ply 2 uniform shirts to each employee at time of hire. Mileage: $0.45 per kilometre (or employee will be paid bus fare) when employee is trans- ferred to another store during work shift.
