Medical benefits: Employer pays 100% of premiums for extended health care plan and supplementary semi-private ac- commodation coverage for em- ployees, spouses and all eligible dependants. Dental: Employer pays 100% of premiums for plan equivalent to Blue Cross #9, up to $3,000 per person per calendar year, with full reimbursement for dental procedures and 50/50 cost share for orthodontics up to $2,000 lifetime maximum ($2,500 as of Jan. 1, 2022; $3,000 on Jan. 1, 2023). Vision: Health-care plan in- cludes coverage of $375 every 24 months (increasing to $400 on Jan. 1, 2022), $125 every 24 months towards eye examina- tion; $1,000 lifetime limit for elective laser correction surgery. Paramedical: Health-care plan includes up to $550 per year for licensed paramedical treatment (increasing to $600 per year on Jan. 1, 2022). Sick leave: 7 days per calendar year. STD: Employer pays 100% of premiums for plan beginning on third day of absence and up to 17 weeks. Number of weeks at full pay increases with each year of service up to all 17 with 10 years of service, with remaining weeks at 66 2/3% of salary. LTD: Employer pays 100% of premiums for coverage of 75% of employee's salary up to $4,000 monthly on first day after STD benefits lapse for first 2 years of disability, for employees not able to perform their own occupa- tions. After 2 years, disability must mean employee cannot perform any occupation. Life insurance: Employer pays full cost of coverage of 2 times annual salary for employee, $10,000 for spouse, $5,000 for child. If employee dies while in employ of employer, benefits will continue for spouse and eligible dependants for 20 months. If employee dies, estate or ben- eficiary will be provided with cheque for $15,000 within 48 hours of request, which will be repaid on receipt of life insurance benefit Pension: OMERS will be provid- ed to employees in accordance with OMERS Act and Regula- tions. Bereavement leave: 5 paid days for death of spouse (including same-sex), parent, stepparent, child, stepchild, sibling, step- sibling. 3 paid days for death of grandparent, grandchild, parent- in-law, sibling-in-law, child- in-law. 1 paid day for death of grandparent-in-law. Up to 2 days can be saved to attend actual in- terment or memorial service. Up to 1 paid day to attend funeral as pallbearer for fellow employees or former employees. Seniority – recall rights: 18 months. Call-in pay: Minimum 3 hours at appropriate overtime rate. $1.50 per hour for each hour em- ployee is on standby. Probationary period: 4 months for full-time employees. 560 to 640 hours or 9 months, which- ever comes first, for part-time employees. Discipline: Sunset clause is 12 months for letter of discipline; 24 months for disciplinary suspen- sion. Safety shoes: $185 per year towards cost of safety boots for each full-time employee requir- ing them and $100 per year for each part-time employee requir- ing them under Occupational Health and Safety Act or by employer. $185 per year towards cost of CSA safety footwear for Rideaucrest home employees. Effective Jan. 1, 2022: $80 to Rideaucrest employees for em- ployer-approved shoes and $110 to bus operators for non-skid black or brown shoes. Uniforms/clothing: Employer will provide special clothing such as rubber boots, rubber suits, safety hats, rain suits and hats, gloves, lined gloves in winter, or such other clothing — in opinion of head of department con- cerned — for employees working on jobs that justify provision of clothing. Certain employees who require safety T-shirts due to visibility sensitive work received safety T-shirts, orange spring jacket, orange winter parka or bomber jacket, orange bib over- alls, orange coveralls or orange hoodie. Certain employees who work mostly outside and jani- tor and print shop employees, receive $250 per year clothing allowance. Garage employees receive 5 coveralls each week provided previous week's issue is returned. Rideaucrest employees receive $180 per year for hospi- tal-type clothing. Bus operators receive pants or shorts, shirts, spring/fall jacket, winter coats, long-sleeved sweater or vest. Tool allowance: $750 per year to mechanics for updating tools and tool chests. $300 per year to apprentice mechanics.
