Shift premium: 70¢ per hour for night shifts. 75¢ effective Oct. 1, 2015. Paid holidays: 10 days. 2 float- ing holidays for employees with 6 months' seniority. Vacations with pay: 2 weeks to start, 3 weeks after 801 days' seniority, 4 weeks after 2,001 days, 5 weeks after 3,001 days and 6 weeks after 5,400 days. Employees will receive an ad- ditional $90 per week for each week of regular vacation taken. Effective Oct. 1, 2015, $95 per week. Overtime: Time and one-half for all hours worked beyond 9 hours per day and for all hours worked on Saturday and Sun- day. Weekly indemnity: The em- ployer will maintain and pay the premium cost of a weekly indemnity insurance plan. Plan provides 70% of weekly earn- ings to a maximum of $620 to commence on the 1st day in case of non-compensable ac- cident and/or hospitalization and on the 4th continuous day from the commencement of a disability due to sickness. Con- tinues for up to 52 weeks, com- bined with UIC benefits. LTD: 55% of monthly earnings to a maximum monthly pay- ment of $2,600. Life insurance: The employer will provide and pay the full cost of an $85,000 life insur- ance and AD&D benefit for a maximum of $85,000 per em- ployee. Bereavement leave: 5 days for employee's spouse, child, father, mother, father-in-law or mother-in-law. 3 days for employee's brother, sister, grandparent, son-in-law, daughter-in-law, brother-in- law, sister-in-law or grandchild. Seniority – recall rights: 2 years. Uniforms/clothing: Employer provides all non-personal safe- ty apparel required. Employees responsible for personal safety apparel including shoes, pants, gloves, corrective lenses and hard hats. Sample rates of pay (current, after 1,5% increase): Bulldozer operator: $28.96 Skidder operator: $28.96 Slasher/processor operator: $28.96 Tractor operator general: $28.45 Loader operator: $29.73 General labour: $27.78 Feller buncher operator: $28.96 Editor's notes: Expenses: Employees will be compensated by a flat allowance of $125 for employment expenses pay- able twice each year on Sept. 1, and on Feb. 1. LONG-TERM CARE Leacock Care Centre Orillia, Ont. (123 employees) and the Health Care and Service Workers Union Local 304, affiliated with the Christian Labour Association of Canada (CLAC) Renewal agreement: Effec- tive April 1, 2013, to March 31, 2016. Signed on March 25, 2015. Wage adjustments: Effective April 1, 2013: 1% lump sum payment Effective April 1, 2014: 1% lump sum payment Shift premium: 15¢ per hour for weekend shifts, 20¢ per hour effective Jan. 17, 2015. Paid holidays: 9 days, plus 3 floaters. No more than 12 paid holidays for each calendar year. Time and one-half for all hours worked on a named holiday. Vacations with pay: 2 weeks after 1 year, 3 weeks after 3 years, 4 weeks after 8 years, 5 weeks after 15 years, 6 weeks after 23 years, 7 weeks after 28 years. Fluoride coverage restricted to persons under 18. Bitewing X-rays every 18 months for adults, every 9 months for children. Full mouth X-rays every 36 months. Vision: $125 every 2 years. $200 every 2 years effective Jan. 17, 2015. Sick leave: 3.75 hours earned for every 162.5 hours worked. AD&D: $20,000. Life insurance: $20,000. Pension: CLAC Pension Plan. 4% contributed by employer, employee can contribute any amount up to 4%. Bereavement leave: 5 days for the death of an employee's child, stepchild or spouse. 3 days for the death of an employee's mother, father, step parents, mother-in-law, father-in-law, brother, sister, brother-in-law, sister-in-law, legal guardian, grandparent, grandchildren, son-in-law, daughter-in-law. 2 days if a funeral cannot be attended, ex- cept in the case of the death of a spouse, child, mother or father. 1 day for the funeral of an aunt, uncle, niece or nephew. Seniority – recall rights: 18 months. Call-in pay: Pay for the entire shift when a call-in is requested one-half hour before start of shift and employee commences work within 1 hour. Minimum 4 hours' pay if the employee reports for work within 1 hour of the request for call-in. Mini- mum 4 hours' pay at straight time rate if an employee re- ports for duty as scheduled but is unable to complete the shift. Probationary period: 450 hours. Discipline: Sunset clause is 1 year for letters of reprimand, 18 months for suspensions. Uniforms/clothing: $6.20 per hour allowance for employees required to wear uniforms. Sample rates of pay, current: Registered practical nurse: $22.86 per hour to start, rising after 4 years to $24.54 Personal support worker: $17.45 to start, rising after 4 years to $19.54 Health-care assistant: $17.45 to start, rising after 4 years to $19.54 Certified activity aide: $17.45 to start, rising after 4 years to $19.54 Certified restorative care aide: $17.45 to start, rising after 4 years to $19.54 Nurse aide: $17.09 to start, ris- ing after 4 years to $18.99 Editor's notes: Influenza immunization: Employees must be vaccinated. Health care employees required to be vaccinated against influenza.
