Part-time employees not eligible for the weekly in- demnity benefit. 66 2/3% of weekly earnings rounded to the next higher $1 to a maximum of $1,000. Payments commence after the elimination period of 3 days for accident or illness. Pension: Employer will con- tribute an amount equal to 25% of the first 6% of wages con- tributed by the employee to the group RRSP. Bereavement leave: 3 days for employee's spouse, father, mother, stepparent, brother, sister, child, father-in-law, mother-in-law, brother-in-law, sister-in-law, grandparent or grandchild. Additional 2 unpaid days available upon request. Seniority – recall rights: 2 years. Call-in pay: Minimum 4 hours' pay. Probationary period: 90 days. Safety shoes: $140 per year. Uniforms/clothing: Employer provides gloves, winter parkas and reflective safety vests as necessary. Sample rates of pay, current: Warehouse: $19.26 per hour Material handler: $19.58 Driver: $20.80 Maintenance: $20.80 Editor's notes: Training: The employer will provide forklift training at least 2 times per year for 5 employees each time and for a minimum of 8 hours. Personal leave: Employees earn 1 unpaid personal day in every 3-month period. HEALTHCARE Extendicare London, Ont. (700 nurses and support service employees) and Unifor Locals 302, 1359, 2458 and 8300 Renewal agreement: Effective Aug. 1, 2014, to July 31, 2016. Signed on March 15, 2015. Wage adjustments: Effective Aug. 1, 2014: 1.5% Effective Aug. 1, 2015: 1.3% Shift premium: 30¢ per hour for all hours worked on the af- ternoon shift. 30¢ per hour for all hours worked on the evening shift. 25¢ per hour for all hours worked on the weekend shift. $7.50 per shift when an em- ployee is required to carry out responsibilities of a salaried em- ployee outside of the bargaining unit for a period in excess of 1/2 their shift. 15¢ per hour for all hours worked in the handyman classification. Paid holidays: 10 days and 1 floating holiday. Vacations with pay: 4% of em- ployee's gross earnings to start, 1 day per month of service to a maximum of 2 weeks after pro- bationary period, 2 weeks after 1 year, 3 weeks after 3 years, 4 weeks after 8 years, 5 weeks after 15 years, 6 weeks after 22 years and 7 weeks after 28 years. Overtime: Time and one-half for all hours worked beyond 7.5 hours per day or 75 hours worked biweekly. Medical benefits: Employer pays 100% of premiums for hearing aid benefit to a maxi- mum of $300. $350 effective July 2016. Dental: Employer pays 50% of premiums to a maximum of $2,000 per employees and per family member. Vision: Employer pays 100% of premiums. $225 every 2 years. $250 every 2 years effective July 2016. Weekly indemnity: Employer pays 100% of premiums for a weekly indemnity plan cover- ing personal illness or injury for weeks 18 through 35. Payment under weekly indemnity will be 66 2/3% of scheduled straight- time wages lost. Sick leave: 3 days after comple- tion of probationary period. Employees accumulate sick leave credits at the rate of 7.5 hours of leave per 162.5 hours worked to a maximum of 105 hours. Life insurance: Employer pays 100% of premiums. $30,000 life insurance plan for full-time em- ployees. $15,000 life insurance plan for part-time employees not enrolled in benefits. Re- duced by 50% after age 65. Pension: Employees contribute an amount equal to 4% of ap- plicable wages. The employer matches such contributions. Bereavement leave: 5 days for employee's spouse, child or stepchild. 3 days for employee's father, mother, stepparent, father-in-law, mother-in-law, brother, sister, brother-in-law, sister-in-law, legal guardian, grandparent, grandchild, son- in-law or daughter-in-law. 1 day for employee's uncle, aunt, nephew or niece. Seniority – recall rights: 3 years. Call-in pay: Minimum 4 hours at time and one-half. Discipline: 2 years for letters of reprimand involving a third party (residents, family, etc.) and 1 year for all other letters of reprimand. 3 years for suspen- sions involving a third party and 18 months for all other suspen- sions. Uniforms/clothing: 7¢ per hour worked. The uniform allowance will not be paid on each cheque but will be accu- mulated and the total annual accumulation will be paid by the last pay period in December of each year. Employer provides protective equipment and clothing as necessary, including gloves, masks and goggles. Sample rates of pay (current, after 1.3% increase): Dietary, laundry and house- keeping aides: $19.22 per hour rising in 2 steps to $20.29 Janitor: $19.22 rising in 2 steps to $20.29 Attendant I, activity aide: $19.42 rising in 2 steps to $20.49 Health-care aide, activity aide certified, PSW: $19.62 rising in 2 steps to $20.68 Cook 1: $21.03 rising in 2 steps to $22.11 Cook 2: $20.64 rising in 2 steps to $21.72 Maintenance: $21.72 rising in 2 steps to $22.70 RPN: $24.52 rising in 2 steps to $25.56 Editor's notes: Layoff: Employer will provide at least 6 weeks' notice in the event of a proposed permanent or long- term layoff to start. 9 weeks after 9 years of service, 10 weeks after 10 years, 11 weeks after 11 years and 12 weeks after 12 years. Pregnancy leave: 17 weeks Parental leave: 13 weeks. Fertility drug plan: $2,500 lifetime maximum prescrip- tion drug coverage for fertility drugs.
