Vacations with pay: 4% to start, 2 weeks after 1 year, 3 weeks after 3 years, 4 weeks after 10 years, 5 weeks after 18 years. No more than 2 weeks to be taken during July and August. Weekly indemnity: 60% of earnings to a maximum of $700 per week to age 75. Payable after first day of accident or hospital- ization and 14th day of sickness for a maximum of 119 days. LTD: 60% of earnings, maxi- mum of $3,000 per month, pay- able after 120 days until age 65. AD&D: $100,000 under 65. $50,000 between 65-75. Life insurance: $100,000 under 65, $50,000 between 65-75. Pension: 3% of employees' gross earnings contributed to the defined contribution CLAC Pension Plan by the employer. Employee can contribute extra through payroll deduction. Bereavement leave: 3 days with pay for death of parent, sister, brother, spouse, child, parent-in-law, stepparent, step- parent-in-law, or stepchild. Premium payments: Employer will pay until end of the month if an employee quits or is laid off or terminated before the 15th of the month. If laid off or terminated after the 15th of the month, employer will pay to the end of the following month. Pension payment: If legislation stops em- ployer from contributing because of employee's age, employer will add equivalent amount to employee's gross pay. License tests: Employer will provide appropriate equipment for the renewal of a license or ticket.
