Vision: Employer pays 100% of premiums to a maximum of $80 ever 2 years for eye exams. LTD: Employer pays 100% of premiums with benefits of 75% of employee's salary commenc- ing 17 weeks after initial date of disability. AD&D: Employer pays 100% of premiums for coverage at the level of 2 times regular salary with a minimum coverage of $100,000. Life insurance: Employer pays 100% of premiums for coverage at the level of 2 times regular salary with a minimum cover- age of $100,000. Bereavement leave: 5 days for employee's father, mother, spouse, child, brother, sis- ter or grandchild. 3 days for employee's father-in-law, mother-in-law, brother-in- law, sister-in-law, son-in-law, daughter-in-law or grandpar- ent. Seniority – recall rights: Em- ployee will accumulate seniority during layoff of 1 year or less. Employee will retain but not ac- cumulate seniority during layoff of more than 1 year.
