Signed on May 20, 2016. Wage adjustments: Effective June 1, 2016: 0.75% Effective Jan. 1, 2017: 0.75% Effective June 1, 2017: 0.75% Effective Jan. 1, 2018: 0.75% Effective June 1, 2018: 0.75% Effective Jan. 1, 2019: 0.75% Shift premium: $38 biweekly for 3 shifts a week working any hours between 6 p.m. and 7 a.m. Language premium: When using language other than Eng- lish, $12 per hour. Paid holidays: 12 days. Vacations with pay: 1 ¼ days per month or 15 days per year to start, 1 2/3 days per month or 20 days per year after 7 years, 2 1/12 days per month or 25 days per year after 15 years, 2 ½ days per month or 30 days per year after 25 years. Overtime: Time and one-half for hours worked after 7 ¼ per day, on regularly scheduled day off or working shift with less than 12 hours between previous shift. Double time for working on second regularly scheduled day off. Double time and one- half for working on a holiday. Meal allowance: Maximum $51 per day (previously $49): breakfast $12, lunch $16, din- ner $23. Medical benefits: 100% premi- ums paid by employer. Dental: 100% premiums paid by employer. Major restorative: 50% co-insurance, $4,000 maxi- mum per year. Orthodontics: 50% co-insurance, $4,500 life- time maximum. Vision: $520 every 24 months (previously $500). $750 per eye once per lifetime for laser sur- gery (previously $675). Paramedical: $1,250 combined maximum each year (previ- ously $1,000) per plan member, spouse, dependant for massage therapy, speech therapy, physio- therapy, occupational therapy, osteopathy, podiatry, chiropo- dy, naturopathy, homeopathy, chiropractic, dietician, nutri- tionist, acupuncture. Clinical psychologist: $2,500 combined maximum (previously $2,000). Effective Jan. 1, 2017, $2,750 per eligible person per year. Cus- tom-made orthopedic shoes: 2 pairs per year, maximum $1,800 per pair STD: Maximum 127 days at 75% of salary. LTD: 100% premiums paid by employer. Lesser of $6,000 monthly or 66 2/3% of salary, payable after 130 days missed. AD&D: 100% premiums paid by employer. Payout of 1 times annual salary, maximum $300,000. Life insurance: 100% premi- ums paid by employer. Payout of 1 times annual salary Pension: WSIB Employees' Pension Plan. Bereavement leave: 5 days for death in immediate fam- ily (spouse, parents, child). For days for death in extended family (brother, sister, parents- in-law, grandparents, grand- children, son-in-law, daughter- in-law). 1 day for death in other family (brother-in-law, sister- in-law, grandparents-in-law). 2 days' travel time for funerals in excess of 600 kilometres away. Seniority – recall rights: 24 months. Probationary period: 180 days. Discipline: Sunset clause is 18 months. Severance: 4 weeks for the first year, 2 weeks for each year up to 8 years, 1 week for each year beyond 8 years, plus attendance credits and vacation credits. Mileage: Effective January 2016: $0.49 per kilometre. Insulin pump: $1,000 one- time purchase. Effective Jan. 1, 2017, maximum $1,250.
