Evening shift: $1.10 per hour. Night shift: $1.05 per hour. Paid holidays: 12 days, plus 1 floater day for the Christmas period. Vacations with pay: 1 day for each month worked, maximum 10 days to start, 2 weeks or 4% after 1 year, 3 weeks or 6% after 3 years, 3 weeks + 1 day or 6.4% after 5 years, 4 weeks or 8% after 9 years, 4 weeks + 1 day or 8.4% after 10 years, 4 weeks + 2 days or 8.8% after 12 years, 4 weeks + 3 days or 9.2% after 14 years, 4 weeks + 4 days or 9.6% after 16 years, 5 years or 10% af- ter 18 years, 5 weeks + 1 days or 10.4% after 20 years, 6 weeks or 12% after 25 years. During the year an employee reaches his 30th, 35th, 40th, 45th and 50th year of service, employee will receive 1 extra day during that year only. Overtime: 1.75 pay for all hours worked after 40 hours (day and evening shifts); 34 hours (night shifts); 30 hours (weekend shifts). Time may be banked with no maximum, but must be paid out by Dec. 15 each year. Meal allowance: $15.50 per shift if employee working at least 1 hour of overtime is not notified during previous shift. Medical benefits: 50% of premiums paid by employer. 100% of generic prescriptions covered. $1,250 per person per year for laboratory tests; $1,000 per person per year for all combined ultrasounds, X-rays, electrocardiograms, mammo- grams, thermograms, MRI and tomography (scans). Dental: 50% of premiums paid by employer. 100% of routine treatment covered, maximum $1,375 per person per year. Ma- jor treatment: 80% reimburse- ment, maximum $2,500 per person, per year. Orthodontic: 75% reimbursement, maximum $2,500 per person, lifetime. Vision: $400 every 2 years, 100% coverage of one eye ex- amination every 2 years. Paramedical: Physiotherapy, osteopath, chiropractic: 100% coverage. Maximum $2,000 per person per year for physio- therapy, additional $5,000 cov- erage if prescribed by physician. Maximum $600 per person per year of combined osteopath, chiropractic coverage. Weekly indemnity: 50% of pre- miums paid by employer. LTD: 100% of premiums paid by employer. 70% of wages cov- ered by plan. AD&D: 50% of premiums paid by employer. Life insurance: 50% of premi- ums paid by employer. Cover- age is twice employee's annual salary. Pension: Participation in com- pany pension plan (employees hired before March 24, 2013) or member-funded pension plan (employees hired after March 24, 2013) is compulsory. Bereavement leave: For death of a spouse, common-law spouse, son or daughter: 10 days off for day and evening- shift workers, 8 days for night- shift workers, 6 days for week- end-shift workers. For death of mother or father: 5 days off for day and evening-shift workers, 4 days for night-shift workers, 3 days for weekend-shift workers. If funeral is more than 500 kilo- metres away, 3 days off to care for children up to the age of 12. Seniority – recall rights: 18 months if less than 2 years' se- niority, 4 years with more than 2 years' seniority. Call-in pay: Minimum 4 hours' pay, 1 hour at 1.75 pay for trav- elling time if not notified the previous day. Probationary period: 480 hours. Mileage: $0.45 per kilometre.
