Prescription safety glasses: $250 every 2 years. National Day of Mourning: Each year on April 28, 1 min- ute of silence will be observed in memory of workers killed or injured on the job. Paid holidays: 10 days. Vacations with pay: 10 days upon completion of 12 months of service; 15 days after 5 years; 20 days after 12 years. No more than 10% of workforce will be granted vacation at one time based on seniority. Overtime: Time and one-half for hours in excess of regular workday and all hours worked on a statutory holiday. Medical benefits: Premiums shared equally between com- pany and employees. Dental: Up to $1,500 annual coverage per family member. 50% coverage for major dental. Pre- miums shared equally between company and employees. Pension: Employees pay 2% of gross pay matched by company to annual maximum of $700. Bereavement leave: 3 paid working days for death of par- ent, spouse, child, grandchild, sibling, parent-in-law, steppar- ent, stepchild. 1 paid working day for death of grandparent or sibling-in-law. Up to 7 addi- tional days without pay may be granted. Probationary period: 5 months. Discipline: Sunset clause is 18 months. Safety shoes: $150 per year al- lowance for safety boots, can be carried over one year. Uniforms/clothing: $250 per year uniform allowance. Com- pany provides coveralls. Mileage: 325 to 450 miles return trip: $0.4258 per mile, rising to $0.43 per mile on May 14, 2017. 451 miles or more return trip: $0.4195 per mile, rising to $0.4279 per mile on May 14, 2017. Sample rates of hourly pay (current): Straight truck driver: $16.60 per hour Tractor/tandem axle truck driver: $18.62 per hour Editor's notes: Overtime refusal: A driver will be removed from the overtime list if he refuses overtime 3 times or fails to show up for scheduled overtime 3 times. Logs review: Drivers must notify the company immediately when their logs are taken for review or copied at any inspection point, or by any government or regulatory body (excluding Canada Revenue Agency). Safety equipment: All vehicles will be equipped with a fire extinguisher. All highway ve- hicles will be equipped with a first-aid kit. Education fund: The company will contribute $500 annually to the Unifor Training Fund for paid education leave.
