Orthodontic coverage and major dental work (including but not limited to crowns, implants, dentures, bridgework): 50% co-insurance, lifetime maximum $3,000 per insured. Vision: $525 every 24 months (previously $500 every 24 months). Effective Jan 1, 2017: $550 every 24 months. Effec- tive Jan 1, 2018: $575 every 24 months. Employer will pay for cost of laser surgery for insured individual, maximum $500 per eye. Paramedical: $800 per year for physiotherapist, clinical psychologist, chiropractor, os- teopath, chiropodist, podiatrist, naturopath/homeopath, speech pathologist, masseur, dietician. Dependants: $600 per person for physiotherapist, clinical psy- chologist, speech pathologist, masseur. LTD: 75% of monthly income, maximum $8,000. Life insurance: Employer pays 100% of premiums for group life insurance at 2 times employee's salary. Pension: O.M.E.R.S. Type 1 pension. Cost shared equally between employer, employee.
