Time and one-half for work on days of rest, double time after 8 hours of overtime. Meal allowance: $17, except where free meal is provided, for employee who works 4 or more consecutive hours of overtime on regular working day. Addi- tional $17 for each subsequent 4-hour period of overtime. Medical benefits: Green Shield Canada plan. Sick leave: 5.625 hours for each month in which employee re- ceived pay for at least 75 hours, maximum 520 hours accumu- lation. LTD: La Capitale Insurance and Financial Services plan. Life insurance: La Capitale In- surance and Financial Services plan. Pension: Employees auto- matically become members of Ontario Municipal Employees Retirement System Plan, which is DB plan, on date of hire. Bereavement leave: 4 days for death in immediate family (spouse, common-law partner, same-sex partner, father-in-law, mother-in-law, child, stepchild, ward, parent, brother, sister, grandparent, grandchild, son- in-law, daughter-in-law; child, parent, brother, sister, grand- parent of employee's spouse or common-law partner, other relative permanently residing in employee's household). Call-in pay: 4 hours at applica- ble overtime rate for each call- in, maximum 10 hours' pay at applicable overtime rate per 10- hour period or applicable rate of overtime for all time worked, whichever is greater. Probationary period: Full winter season's operations or 6 months service, whichever is greater for operations staff; 6 months for all other newly hired employees. Discipline: Sunset clause is 12 months for level 1 disciplin- ary action (formal letter of rep- rimand, notice on file of verbal warning but no suspension); 24 months for level 2 disciplin- ary action (suspension without pay for up to 5 days); 48 months for level 3 disciplinary action (suspension without pay in ex- cess of 5 days). Severance: After 12 months of employment, 2 weeks' pay for first year of employment; 1 week's pay for each additional year. Safety shoes: $175 per year for approved safety footwear. $50 per year to café, lounge employ- ees for non-slip safety shoes. Uniforms/clothing: Employer will supply employees with uni- forms, required clothing, when employees are required to wear uniforms. Paid holidays: 10 days, plus 1 floating holiday. Vacations with pay: 1 day per month, maximum 2 weeks or 4%, to start, 2 weeks or 4% af- ter 1 year, 2 weeks or 5% after 3 years, 3 weeks or 6% after 5 years, 4 weeks or 8% after 12 years, 5 weeks or 10% after 20 years. Overtime: Time and one-half for all work outside weekly posted schedule. Meal allowance: Maximum $20 for meal if employee works, without advance notice, for more than 2 additional hours outside regular work hours. Medical benefits: Employer pays 50% of premiums, employ- ee pays 50% of premiums. Dental: Employer pays 50% of premiums, employee pays 50% of premiums. 80% coverage for minor dental care, services. Maximum $1,000 per person per year. STD: Coverage of 66.67% of weekly salary, maximum $400 per week, maximum 26 weeks. LTD: Coverage of 50% of monthly salary, maximum 85% from all sources. AD&D: Employer pays 50% of premiums, employee pays 50% of premiums. Life insurance: Employer pays 50% of premiums, employee pays 50% of premiums for cov- erage of $20,000. Pension: Employer will match employee's contribution, maxi- mum 1% in group retirement saving plan.
