Vacations with pay: 15 days (1.25 days per month) to start, 20 days (1.67 days per month) af- ter 8 years, 25 days (2.08 days per month) after 15 years, 30 days (2.5 days per month) after 21 years. Additional 5 days (to be taken within 12-month period) after 25 years. Maximum 1 year's entitlement may be carried over into following year. Medical benefits: Employer pays 100% of premiums for current health benefits plan and employee and family assistance plan. Dental: Employer pays 100% of premiums. 85% coverage for preventative. 60% coverage, maximum $2,500 per year, for major restoration (crowns, bridges, dental implants). 50% of costs, lifetime maximum $5,000, for adults and children for orthodontic. 75% of costs, lifetime maximum $5,000, for prosthodontic services (dentures). Vision: Employer pays 100% of premiums for coverage of prescription eyewear max- imum $400 over 2-year period, including cost of 1 eye exam- ination within 2-year period ($400 over 2-year period can be allocated for laser eye surgery). Paramedical: Coverage of $1,200 per practitioner per year for Ontario-licensed chiro- practor, osteopath, chiropodist, podiatrist, naturopath, speech therapist, masseur, physiother- apist, acupuncturist. Coverage of $2,000 per year for psych- ologist for psychotherapy and testing. Sick leave: 100% pay for 6 days per year (unused days do not carry forward). STD: Employer pays 100% of premiums for coverage of 75% pay for 124 days per year. LTD: Employer pays 85% of premiums, employee pays
