Signed on May 1, 2019. Shift premium: 10% for local hourly work on weekends. $10 for use of tarps in-town (5-ton trucks only). $10 per hour for Hiab crane. Paid holidays: 11 days. Medical benefits: Owner-operators pay 100% of premiums. Employer will cover all employees (after 90 days from start date) in health and welfare plan and abide by con- ditions of Unifor Benefit Trust, administered by Bilsland Grif- fith Benefit Administrators. Dental: 100% reimbursement of fees for basic services. 80% reimbursement for member, spouse, eligible dependent children older than 18, (100% for dependent children younger than 19) for major services. 50% reimbursement for all eligible persons for orthodontic services. Vision: $200 per person in any 24-month period for prescription eyeglasses, prescription contact lenses. $50 in any 24-month period for eye examination by licensed optometrist. Paramedical: $30 per visit, maximum $350 per person, $750 per family per calendar year (testing fees, X-rays and medication excluded) for natu- ropath/physiotherapist/mas- sage therapist/speech therapist. Maximum benefit of $150 per person per calendar year for acupuncturist. $10,000 per year for registered nurse when re- ferred (must not be relative or residing with employee). Weekly indemnity: Coverage of 75% of pre-disability earn- ings, maximum $435 per week. LTD: Flat monthly benefit of $1,000. AD&D: Coverage of $60,000. Life insurance: Coverage of $50,000. Seniority – recall rights: 1 year; 18 months after 5 years of seniority; 120 days when owner-operator requests layoff or leave of absence. Hearing aids: Maximum $500 for each ear during any 36-month period. Treatment for substance abuse: Plan will pay 100% of treatment fees in residential treatment centre, licensed by B.C. or Yukon territory at nor- mal cost for such treatment as recognized by those governments, maximum benefit of $1,500.
