Vacations with pay: 1 day per month of completed service after 2 months, 2 weeks after 1 year, 3 weeks after 2 years, 4 weeks after 5 years, 5 weeks after 12 years (previously 13 years), 6 weeks after 21 years (previously 22 years), 7 weeks after 27 years (previously 28 years). Overtime: Time and one-half for time worked in excess of 7.5 hours in any 1 day or 75 hours in any biweekly pay peri- od. Time and one-half for each shift worked without 12-hour rest period and on seventh and subsequent consecutive days. Time and one-half for all hours worked on second consecu- tive weekend when employee is normally off or on statu- tory holiday. Double time and one-half for hours worked on statutory holiday in excess of 7.5 hours. Meal allowance: $7 meal voucher when employee works more than 2 hours of overtime contiguous to regular shift or if employee is requested to work overtime with less than 8 hours of prior notice. Medical benefits: Employer pays 100% of premiums for Green Shield semi-private plan. Dental: Employer pays 75% of premiums, employees pays 25% of premiums for Green Shield Plus 3 dental plan. Orthodontic coverage will be provided on 50/50 coinsur- ance, lifetime maximum of $1,500 per insured person. $4,000 lifetime maximum for crowns, bridges. Vision: Employer pays 100% of premiums for vision plan. $400 every 24 months. $90 for eye exams every 24 months. Sick leave: 3 paid days at end of probationary period plus 1 day for each additional month of employment. 1.5 paid days per month after 1 year of em- ployment. No sick pay benefit is payable for first 15 hours of sixth and subsequent periods of absence in same fiscal year. Life insurance: Employer pays 100% of premiums for Hos- pitals of Ontario Group Life Insurance Plan for coverage of 2 times annual salary. Pension: Healthcare of On- tario Pension Plan (HOOPP) Bereavement leave: 4 sched- uled working days for death of spouse, child, parent, sibling, sibling-in-law, child-in-law, parent-in-law, grandparent, or grandchild. 3 scheduled working days for death of great grandparent, spouse's grand- parent, stepparent of employee or spouse, stepchild, stepsib- ling, legal guardian. 1 working day for death of employee's or spouse's aunt, uncle, niece, nephew. 1 additional day for burial or memorial service if held within 6 months of date of bereavement. Seniority – recall rights: 30 months; 2 years when employee is absent because of illness or injury (further 6 months for employee who was injured on job and covered under Workplace Safety and Insurance Act). 6 months for employee who is member of IBEW and who permanently transfers into position in hos- pital that is not covered by terms of collective agreement or when transfer is for tempo- rary period. Call-in pay: Minimum 4 hours' pay when reporting for scheduled work and no work is available. Probationary period: 60 working days in consecutive 6-month period of employ- ment. Discipline: Sunset clause is 18 months for reprimands or disciplinary action. Severance: If an employee resigns within 30 days after receiving notice that position will be eliminated, 2 weeks' salary for each year of con- tinuous service to maximum of 12 weeks' pay plus maxi- mum $3,000 to take approved educational program within 12 months of resignation. 2 weeks' pay for each year of service maximum of 26 weeks for retirement after notice of permanent or long-term layoff plus lump-sum payment of $1,000 for every year younger than 65, maximum $5,000.
