COLLECTIVE AGREEMENTS COLLECTIVE AGREEMENTS berta Hospital and Medical Care Health Care Insurance Plan. Em- ployer pays 100% of premiums for major medical plan. Dental: Employer pays 100% of premiums. 100% for basic; 50% for major restorative, orth- odontia. Maximum $2,000 per person per year for basic, major restorative. Lifetime maximum $2,500, with annual maximum of $1,250.00 (at 50%) for orth- odontia. Vision: Employer pays 100% of premiums for coverage of $225 in any 2 years for frames, lenses, fitting of prescription glasses, sunglasses, including contact lenses or additional $100 for prescription glasses only if pre- scription changes after 1 year per family member. Paramedical: 80% coverage, maximum $400 per year for massage therapy from registered massage therapist or treatment from registered chiropractor. 80% coverage, maximum $1,000 per year for physiotherapy. Weekly indemnity: Employer will pay cost for 80% of maxi- mum EI insurable earnings, minimum $800 per week, after first day of accident, fourth day for sickness, first day of hospital- ization, maximum 26 weeks. LTD: Coverage of $1,200 per month or benefit provided under individual's pension calculation, whichever is greater. AD&D: Employer pays 100% of premiums for coverage of $50,000. Life insurance: Employer pays 100% of premiums for coverage of $50,000. Pension: Employees will contribute $0.25 per hour for all hours worked, including hours paid for vacation, statu- tory holiday, weekly indemnity, maximum contribution of $450 per year into pension plan. Em- ployees will also contribute 5%, matched by employer, after 120 days of employment, into DC plan. Bereavement leave: 5 days for death of wife, husband, children, all corresponding common-law equivalents. 3 days for death of mother, father, brother, sister, grandparents, grandchildren, brother-in-law, son-in-law, daughter-in-law, sister-in-law, mother-in-law, father-in-law, all corresponding common-law equivalents. Seniority – recall rights: Pe- riod of employment to start; 12 months after 12 months of service; 18 month after 3 years of service, for continuous layoffs. 18 months for employees off as result of illness or injury. Call-in pay: Minimum 4 hours of straight-time pay (except where failure to so is because of non-operation of plant; substan- tial part thereof, as result of fire, act of God, failure of power or major breakdown of equipment). Double time when called out prior to shift. Probationary period: 120 cal- endar days within 12 months from first date of employment. Discipline: Sunset clause is 12 months. Safety shoes: $200 per year to each employee for approved safety boots. Editor's notes: Performance bonus plan (PBP): Employer will guarantee minimum payout level of productivity por- tion at $1.65 per hour on bonus plan. Physician fees: When employee is required to get signed document from employ- ee's personal doctor by either employer or its insurer, employer will reimburse employee for reasonable fees incurred.
