COLLECTIVE AGREEMENTS COLLECTIVE AGREEMENTS w/tool box 3. $2.25 per hour for chief steward w/tool box 1. $2.50 per hour for chief stew- ard w/tool box 2. $3 per hour for chief steward w/tool box 3. $5 per hour when welding al- loys for all JM and apprentices. Paid holidays: 11 days. Vacations with pay: Employer will consider vacations at times requested considering business requirements. 6% of base wage rate for all hours worked will be paid to employ- ees on each paycheque. Overtime: Time and one-half for all work after 8 hours per day or 40 hours per week. Meal allowance: Meal period of one-half hour and meal for employees (or one-half hour of pay in lieu of meal and break) if employees are not scheduled but work beyond 12 hours in 1 day. Medical benefits: Employer contributes $1.99 per hour (previously $1.45 per hour) into CLAC Health and Wel- fare Trust Fund. Coverage be- gins after 350 hours worked. Dental: Coverage of 100% reimbursement, maximum $2,000 per person annual for basic services; 50% re- imbursement, maximum $2,000 per person per year for comprehensive; 50% reim- bursement, maximum $3,000 lifetime maximum per child younger than 19 for orth- odontic. Vision: Coverage of $300 per year for dependants younger than 21; $300 every 2 years for those 21 and older. Paramedical: $50 per visit, doctor's referral required, for massage therapy. STD: Coverage of 60% weekly basic earnings maximum $600 per week, payable after first day of accident or hospitaliza- tion; 14th day of illness maxi- mum 119 days. LTD: Coverage of 60% of weekly basic earnings maxi- mum $2,600 per month, pay- able after 119 days. AD&D: Coverage of $100,000. Life insurance: Coverage of $100,000. Pension: Employer, employee will contribute 4% to RSP plan. New employees will join CLAC pension plan immedi- ately. Employer will contribute matching 2% to pension plan, for each employee, for all hours worked. Each employee will also contribute 2% match- ing amount to pension plan for all hours worked. Bereavement leave: 3 days for death of spouse, common-law spouse, child, father, mother, father-in-law, mother-in-law, brother, sister, grandparents, grandchildren. Further time may be given when requested. Call-in pay: Minimum 2 hours' pay at appropriate rate. Probationary period: 90 cal- endar days. Safety shoes: All employees will wear safety shoes where required supplied by em- ployee. Uniforms/clothing: All em- ployees will wear safety hats to be made available by employer. Employer will supply employ- ees with safety equipment in- cluding gloves, safety glasses, raingear, particulate masks, breathing apparatuses, fall-ar- rest equipment, fire-retardant coveralls when required.
