Wage adjustments: Effective Nov. 16, 2019: 1.1% Effective Nov. 16, 2020: 1.1% Effective Nov. 16, 2021: 1.1% Shift premium: $0.45 per hour for all hours worked in shift where majority of scheduled hours fall between 3 p.m. and 7 a.m. $2.10 per full evening or night shift for part-time em- ployees who regularly rotate through shift schedule. $0.50 per hour for each hour worked between 11 p.m. Friday and 11 p.m. Sunday. Employee who is regularly scheduled to work night shift in home will receive 1.25 day's pay per quarter (full- time) in lieu of not being able to leave for lunch break. 0.625 day's pay per quarter for part- time employee. $1 per hour or $7.50 per shift for employee who is in charge of centre, when there are no supervisory staff present. Paid holidays: 13 days includ- ing 1 float day for full-time employee's birthday or first Monday in April for part-time employee. Employer will pro- vide for scheduling off of full- time employees for 3 consecu- tive days at either Christmas or New Year's and will make every effort to provide 4 consecu- tive days, except for employees who work Monday to Friday or are not normally scheduled to work on paid holiday. Vacations with pay: 1 day for each completed month of service up to 9 days to start, 2 weeks after 1 year, 3 weeks after 3 years, 4 weeks after 8 years, 5 weeks after 15 years, 6 weeks after 25 years. Seating technol- ogists, kinesiologists, occupa- tional therapists, social workers receive 4 weeks after 1 year. Overtime: Time and one-half for all hours worked in excess of 7.5 in any 1 day or 75 hours in bi-weekly period. Not appli- cable to full-time staff working in regularly scheduled night positions, exchange of shifts, requested overtime by employ- ee for convenience or change to daylight saving time and vice versa. Medical benefits: Employer pays 75% premiums, employee pays 25% of premiums for ex- tended health care plan. Dental: Employer pays 75% of premiums, employee pays 25% of premiums for dental plan. Vision: Employer pays 100% premiums for coverage of $200 plus eye examination every 24 months. Sick leave: Employer pays 75% of premiums, employee pays 25% of premiums for Hospitals of Ontario Disability Income Plan (HOODIP). Life insurance: Employer pays 100% premiums for Hospitals of Ontario Group Life Insur- ance Plan (HOOGLIP). Pension: All full-time em- ployees are enrolled in Hospi- tals of Ontario Pension Plan (HOOPP). Bereavement leave: 4 paid days for death of spouse, child, stepchild, parent, sibling, child- in-law, parent-in-law, sibling- in-law, grandparent, grand- child, guardian, stepparent, stepgrandparent, foster child. 1 paid day for death of great- grandparent, aunt, uncle, niece, nephew. 1 extra day without pay will be allowed when funer- al is more than 480 kilometres from location of employer. Seniority – recall rights: 24 months for layoffs or when em- ployee is absent due to illness or disability. Call-in pay: Minimum 4 hours at time and one-half if called in outside of regular sched- uled hours after leaving work, except if call-in overlaps into regular shift. Probationary period: Full- time employees: 90 days worked; may be extended for further 90 days. Part-time employees: 675 hours worked, may be extended further 675 hours. Discipline: Sunset clause is 18 months for letters of reprimand or suspension. Safety shoes: $100 per year for full-time employees and $75 per year for part-time employ- ees for appropriate footwear as reasonably determined by employer. Maintenance and seating techs receive $125 per year for safety shoes. Uniforms/clothing: Employer supplies and launders uniforms or provides uniform allowance of $60 per year. Mileage: $0.45 per kilometre when employee is authorized to use personal vehicle while on employer business.
