(448 health care and support employees) and the Southwestern Ontario Health Care and Service Workers, CLAC Local 303 Renewal agreement: Effec- tive April 1, 2019 to March 31, 2022. Ratified in October 2020. Signed on April 1, 2019. Wage adjustments: Effective April 1, 2019: 1% Effective April 1, 2020: 1% Effective April 1, 2021: 1% Shift premium: $1.25 per hour for employees regularly performing shifts commenc- ing on or after noon and end- ing after 5 p.m. $1.30 per hour for each hour worked between midnight Friday and midnight Sunday. Time and one-half for all hours worked on next shift when shift is cancelled with less than 24 hours of notice (12 hours of notice for part-time employees). Paid holidays: 12 days. Vacations with pay: 5/6 of 1 day per each completed month of service to start, 2 weeks after 1 year, 3 weeks after 2 years, 4 weeks after 5 years, 5 weeks after 12 years, 6 weeks after 21 years, 7 weeks after 28 years. Overtime: Time and one-half for all work performed in ex- cess of 7.5 hours per day, 37.5 hours per week, on scheduled day off (if not at employee's request), in excess of 7 con- secutive days, on third and sub- sequent consecutive scheduled shifts, on third and consecutive scheduled weekend, on statu- tory holidays, and in excess of 11.25 hours on 1 shift in unit/ department/classification where extended tours are pres- ent. Double time for authorized overtime on paid holiday. Meal allowance: $6 if employ- ee is held over and required to work more than 2 hours be- yond standard workday. Medical benefits: Employer pays 75% of premiums for extended health care plan, em- ployee pays 25% of premiums. Employer pays 100% of premi- ums for semiprivate coverage. Dental: Employer pays 75% of premiums for plan at current ODA schedule of fees, em- ployee pays 25% of premiums. $2,250 per lifetime for orth- odontic coverage. Vision: Employer pays 75% of premiums, employee pays 25% of premiums for plan providing $300, plus 1 eye exam, every 2 years. Paramedical: Employer pays 75% of premiums, employee pays 25% of premiums for cov- erage of $325 per speciality, per calendar year. Sick leave: Paid sick leave until first 15 hours of absence for fourth and subsequent periods of absence in same fiscal year. LTD: Employer pays 75% of premiums, employee pays 25% of premiums under Hospitals of Ontario Disability Income Program (HOODIP). Life insurance: Employer pays 100% of premiums of premi- ums. Pension: Enrolment in pension plan is condition of employ- ment. Bereavement leave: 4 con- secutive paid days for death of spouse, child, parent. 3 con- secutive paid days for death of sibling, stepchild, parent-in- law, sibling-in-law, grandchild, grandparent, spouse's grand- parent, spouse's grandchild. 1 paid day for funeral of step- sibling, niece, nephew, legal guardian of employee, aunt, uncle. 1 day can be used within 6 months if burial or memorial service occurs later. Employer may extend such leave without pay at its discretion. Seniority – recall rights: Greater of 6 months or em- ployee's seniority for employees with less than 2 years of senior- ity; 30 months for employees with more than 2 years of se- niority. 45 days for posting out of bargaining unit (18 months for temporary transfers). Call-in pay: Minimum 4 hours' pay at time and one-half for employees on standby who are called back to work. Double time for hours worked in excess to hours of normal shift. Probationary period: 2 months for full-time employ- ees; 450 hours within any 12-month period for part-time employees. Discipline: Sunset clause is 18 months. Severance: Greater of 2 weeks' pay per year of service up to 26 weeks' pay for full-time em- ployee laid off for 26 weeks in any 52-week period. Safety shoes: $100 per calen- dar year for purchase of CSA- approved safety footwear. Uniforms/clothing: $9-per -month allowance for employ- ees required to wear uniform. Maintenance staff will have uniforms supplied and laun- dered. Early retirement allowance: Before issuing notice of long-term layoff, offers of early retirement allowance in order of seniority to employee eligible for early retirement under HOOPP, equal to 1 week's salary for each year of service up to 35 weeks' salary. Compression stockings: 1 pair every 6 months, maximum $75 per pair. Education fund: Employer agrees to pay $3,000 per year to union's education fund.
