Steam tickets: For all employees with third-, second-, first-class steam tickets paid out on quarterly schedule (January, April, July, Octo- ber): $2,400 every 3 months for first-class ($9,600/year); $1,800 every 3 months ($7,200/year) for second-class; $600 every 3 months ($2,400/year) for third-class. Mandatory courses: Employees required to attend courses on regular days off that are deemed mandatory by employer will be paid at respective overtime hourly wage rates of pay for hours of attendance. Post-plant-closure costs: Employer will reimburse employee for training or moving costs incurred within 2 years of termina- tion, maximum $2,000, provided such expense is for outside employment opportunity, less any other training or moving subsidy available to employee. Training costs will include registration, tuition fees, books, examination fees. Safety glasses: Employer will provide safety glasses to employees as per company protective-glass standards. Travel allowance: $35 per round trip for employees who must travel abnormal distance on daily basis to plant site on own time. Paid education leave: Employer will remit $0.05 for each full-time employee's regular hours of work for paid education leave. Boiler association examinations: Overnight accommodation will be reimbursed if employees stay at approved hotel on employer's accommo- dation list. If employees choose to stay with family or friends, $20 dollars will be reimbursed. Per diems will be reimbursed at $50 per day. Parking expenses will be reimbursed as required. Community fund: Employer will remit $0.03 for each full-time employee's regular hours of work to Unifor Canadian Community Fund. LONG-TERM CARE HOMES St. Joseph's Villa Dundas, Ont. (570 residential care workers) and the Canadian Union of Public Employees (CUPE), Local 1404.00 Renewal agreement: Effective July 3, 2019 to July 2, 2022. Rati- fied in January 2021. Signed on April 21, 2021. Wage adjustments: Effective July 3, 2019: 1% Effective July 3, 2020: 1% Effective July 3, 2021: 1% Shift premium: 2% of regular hourly rate per hour for each shift worked between 2:30 p.m. and 6:30 a.m. $0.25 per hour for all hours worked between Fri- day at 10:30 p.m. and Sunday at 10:30 p.m. $0.40 per hour dur- ing assignment for employee assigned to work in charge function. Paid holidays: 12 days for full-time employees, 9 days for part-time employees. Employer will endeavour to schedule so that employees will be off duty either New Year's Day or Christmas Day. Vacations with pay: 0.833 days per each completed calendar month up to 10 days to start, 1.25 days per completed calen- dar month up to 15 days after 3 years, 1.667 days per completed calendar month up to 20 days after 8 years, 2.083 days per completed calendar month up to 25 days after 15 years, 2.5 days for each completed calen- dar month up to 30 days after 22 years, 2.917 days per calen- dar month up to 35 days after 28 years. Overtime: Time and one- half for any hours worked in excess of 7.5 per day or 75 in 2-week pay period, and all hours worked on recognized holiday. Medical benefits: Employer pays 100% of premiums for extended health care plan. Dental: Employer pays 50% of premiums, employee pays 50% of premiums for group dental plan (exam and cleaning every 9 months). Vision: Employer pays 100% of premiums for coverage of $300 every 2 years (previously $250 every 2 years) and cost of 1 ex- amination every 2 years. Sick leave: 11.25 hours per completed month of employ- ment with less than 1 year of continuous full-time employ- ment. After 1 year, 135 hours per calendar year. Can be accu- mulated up to 1,500 hours. Life insurance: Employer pays 100% of premiums for coverage equal to twice employee's an- nual salary. Pension: Hospitals of Ontario Pension Plan (HOOPP). Bereavement leave: 5 paid shifts for death of spouse (in- cluding common-law, same- sex); fiancée, child, parent. 4 paid shifts for death of sibling, parent-in-law, child-in-law, grandparent, grandchild, sib- ling-in-law. Seniority – recall rights: 18 months. Call-in pay: Minimum 3 hours' pay at appropriate rate. $25 per day for each full or partial day spent on call. Probationary period: 45 working days. May be extended with written consent of em- ployer, probationary employee, and president of local union or designate. Discipline: Sunset clause is 18 months. Safety shoes: $145 every 2 years to each full-time em- ployee who is required to wear safety footwear as condition of employment. Uniforms/clothing: Employ- ees required to wear uniform as condition of employment are paid $0.20 per hour, to in- crease by $0.01 in each year of contract. Tool allowance: $0.20 per hour to mechanic classifications. Mileage: Employees who are required to use personal ve- hicle for employer business will be reimbursed $0.42 per kilometre or at Villa Policy rate, whichever is greater, for all ki- lometres driven between work locations.
