Vacations with pay: 2 weeks af- ter 1 year, 3 weeks after 5 years, 4 weeks after 10 years, 5 weeks after 17 years, 6 weeks after 25 years. Overtime: Time and one-half for all hours worked in excess of regularly assigned daily hours. Double time for all overtime hours in excess of 8 on any 1 day of rest, all overtime hours on second and subsequent days of rest; all overtime hours on Sun- days. Double time for all hours worked on statutory holiday. Medical benefits: Employer pays full cost of prescription drugs, 80% of general and medi- cal expenses, full semi-private hospital coverage. Dental: Employer pays 100% of premiums for coverage for 90% of basic treatment, employer pays 50% of premiums, employee pays 50% of premiums for cover- age for 80% of major restorative, and 50% of orthodontics. Vision: Employer pays 100% of premiums for coverage for $400 every 24 months for prescrip- tion glasses or contact lenses and $200 every 24 months towards eye examination and digital retinal photos. Paramedical: Employer will pay up to $2,000 for hearing aids every 24 months; 50% of cost of prosthetic appliances; 80% of cost of durable medical equipment up to $2,500; up to $1,000 for smoking cessation assistance; up to $1,500 per year for massage, chiropractic, naturopathy, podiatrist visits; 70% for osteopath visits, 80% of speech therapy up to $2,000, full cost of PSA tests. Weekly indemnity: 66 2/3% of employee's straight-time rate for 40 hours per week, during pe- riods of disability from first day of absence due to non-occupa- tional accident, hospitalization, outpatient hospital care or third day of absence due to illness, up to 32 weeks. For employees until they reach 70 years of age. LTD: Employer pays 100% of premiums for coverage of 50% of regular pay rate, beginning after expiry of weekly indemnity period of 32 weeks, for addi- tional period of up to 24 months when employee is unable to per- form any and every duty of any occupation in mine. AD&D: Employer pays 100% of premiums for coverage for 3 times employee's straight-time rate multiplied by 2,080 and ad- justed to nearest $100. Seniority – recall rights: 12 months for employees with less than 1 year of service, 24 months after 1 year of service, 36 months after 10 years of service, 48 months after 20 years of service. Probationary period: 720 hours worked. Discipline: Sunset clause is 12 months for written discipline or counselling notices, 24 months for suspensions. Severance: Permanently laid off employees with 5 or more years of service receive 2 week's pay for each year of service plus pro- rated amount for partial years of service. Safety shoes: $300 maximum reimbursement for cost of re- placing worn or damaged boots meeting specified level of pro- tection (previously $200). Uniforms/clothing: Company pays 50% of cost of 1 pair of safe- ty prescription glasses per year. Company pays for and provides weekly rental and cleaning of personal uniform services for 5 coveralls, 5 sets of pants and shirts for all employees; person- al lined coveralls and insulated coats for posted employees working out of doors during winter months.
