(250 office employees) and Unifor Local 2458 Renewal agreement: Effective July 1, 2013, to June 30, 2016. Signed on Sept. 14, 2014. Wage adjustments: Effective July 1, 2013: 1.5% Effective July 1, 2014: 1.5% Effective July 1, 2015: 1.5% Shift premium: 65¢ per hour for shifts starting after 11:59 p.m., 72¢ per hour for shifts starting after 7:59 p.m. and 90¢ per hour for time worked on Saturday and Sunday. Paid holidays: 10 days. Vacations with pay: 10 days af- ter 1 year, 15 days after 3 years, 20 days after 10 years, 25 days after 19 years, 26 days after 20 years, 27 days after 21 years, 28 days after 22 years, 29 days after 23 years and 30 days after 24 years. Overtime: Double time for all hours worked in excess of the normal daily and weekly hours and for all hours worked on the first or second regularly sched- uled day off. Meal allowance: A meal, hot where possible, not exceeding $10 provided by the employer where employees are required to work more than 2 hours overtime. Medical benefits: Health plan includes generics substitution drug plan with $3 co-pay for each prescription drug. Mas- sage therapy to a maximum of $800 per year. Physiotherapy to a maximum of $800 per year. Chiropractic to a maximum of $600 per year. Private room coverage of $5,000 per year. PSA test annually. C125 test annually. Hearing aid coverage at $1,000 every 5 years. Private duty nurse at $20,000. Dental: $3,5000 orthodontic maximum. Dental check-ups every 9 months. Major restor- ative to a maximum of 80%. Vision: $400 and 1 eye exam every 2 years. Vision care ben- efit can be applied to laser eye surgery. aD&D: Employer pays 100% of premium rate. Life insurance: 2 times the em- ployee's annual base salary to a maximum of $150,000. Bereavement leave: 5 days for employee's legal or common- law spouse, son, daughter, step-son, step-daughter or ward of the employee. 3 days for em- ployee's father, mother, brother, sister, father-in-law, mother- in-law, son-in-law, daughter- in-law, brother-in-law, sister- in-law, stepfather, stepmother, grandparent, grandchild or grandparent of the employee's legal or common-law spouse. Seniority – recall rights: 2 years. Employees maintain full benefit coverage for a period of 18 months. Call-in pay: 4 hours at regular straight time or actual hours worked at applicable overtime rate, whichever is greater. Probationary period: 90 days. Discipline: Sunset clause is 2 years. Bereavement leave: 3 days for employee's father, mother, brother, sister, grandparent, grandchild, father-in-law, mother-in-law, son-in-law, daughter-in-law, legal guardian or relative residing with the em- ployee. 1 additional day when the ceremony occurs outside the province. 1 day for employ- ee's uncle, aunt, nephew, niece, brother-in-law or sister-in-law.
