Dental: 100% coverage up to $2,000 per person per year for basic services. 50% cover- age up to $2,000 per person per year for comprehensive services. 50% of coverage up to $3,000 lifetime maximum per child under 19 for orthodontic services. Vision: $300 per year for those under 21, $300 per year for those over 21. StD: 60% of weekly earnings up to $600 per week for up to 119 days. ltD: 60% of earnings up to $2,600 per month, to start af- ter 119 days. aD&D: $100,000 per em- three paid days if an incident should occur within the immediate family that requires arrangements made by the employee.
