Vision: Gold plan: $300 per year if under 21, $300 every 2 years if over 21. STD: 60% basic weekly earn- ings up to $600 per week. LTD: 60% of basic monthly earnings up to $2,600 per month. Bereavement leave: 3 days for the death of an employee's spouse, common-law spouse, child, legal dependant, father, mother, brother, sister, legal guardian, mother-in-law, father-in-law, grandparents or grandchildren. Seniority – recall rights: 6 months. Call-in pay: Minimum 2 hours' pay at prevailing hourly rate for employees who report for duty but find no work avail- able. Minimum 4 hours' pay at prevailing rate or number of hours worked multiplied by the prevailing hourly rate (which- ever is greater) for employees who commence work. Mini- mum 2 hours' pay at appropri- ate rate for employees called back to work in the same day. Probationary period: 3 months. Safety shoes: $200 per year, to be paid first after 6 months of employment. Uniforms/clothing: Employer to supply safety hats, gloves, hearing protection, non- prescription safety glasses, shields, goggles, fire retardant coveralls, rain gear, particulate masks, breathing apparatuses and fall arrest equipment. $300 for the cost of prescription safety glasses, or laser eye sur- gery. Tool allowance: Tradesmen to supply tools common to their trade, specialty tools provided by employer. Mileage: 50¢ per km for em- ployees whose permanent place of residence between 55 and 80 km from base for travel between the employee's resi- dence and the 55-km radius line. Probationary period: 3 months. Uniforms/clothing: Up to $500 per year for the purchase of safety boots, tools or work- wear. Unclaimed allowance will not carry forward. Up to $400 per year for employees with less than 3 years of service. Tool allowance: All trades- men to supply their own hand tools. Consumable items, such as drill bits, to be replaced as needed by employer. Up to $500 made available as inter- est-free loan from employer for employees to purchase tools, repaid through paycheck re- duction of $25 per pay period. Mileage: Free travel zone for all jobs inside GTA, including Toronto, York and Peel regions, Durham and Halton. When travel for a job is required be- yond the free zone, and 50 km beyond employee's residence, employer and union to meet and discuss appropriate travel pay.
