Time and one-half premium for hours worked without 8-hour break between shifts. Paid holidays: 11 days. Vacations with pay: 2 weeks to start, 3 weeks after 4 years, 4 weeks after 9 years. Overtime: Time and one-half for all hours over regular sched- ule. Double time for working a statutory holiday. Medical benefits: Company covers 90% of drugs, 100% of others. Dental: Company covers 100% of diagnostic and preventative, 80% of minor restorative, 50% of major restorative, and 50% of orthodontic for dependent children under 18. Maximum of $1,500 per year, per category. Vision: $250 every 2 years for prescription safety glasses. $250 every 2 years for vision care. Paramedical: $20 per visit per practitioner up to a $300 maximum. Includes psycholo- gist, podiatrist, chiropractor, osteopath, naturopath, phys- iotherapy, speech therapy and masseur. Weekly indemnity: Current EI to $600 for a maximum of 15 weeks, starting immediately after accident or hospitalization and 7 days after illness. Sick leave: 6 sick days per year — 3 days given on Jan. 1 and 3 more on July 1. LTD: 60% coverage up to $2,500 for up to 15 weeks. AD&D: $30,000, dropping to 50% of that at age 65. Life insurance: $30,000, drop- ping to 50% of that at age 65. Pension: Company contrib- utes $1.25 per hour (previously $1.20). Bereavement leave: 5 days for death of employee's spouse, mother, father, sons, daughters, brothers, sisters, mother-in- law, father-in-law, grandpar- ents, and legal guardian. Seniority – recall rights: 1 year. Probationary period: 3 months. Safety shoes: $125 per year for approved safety boots. Uniforms/clothing: Uniform provided by company. Compa- ny will provide personal protec- tive equipment as needed. TRANSPORTATION Sysco Central Ontario Peterborough, Ont. (250 warehouse employees) and Teamsters Canada (CtW/CLC) Renewal agreement: Effective March 19, 2016, to March 18, 2021. Signed on May 18, 2016. Wage adjustments: Effective March 19, 2017: 2.5% Effective March 19, 2018: 3% Effective March 19, 2019: 3% Effective March 19, 2020: 3% (Calculated by CLR.) Shift premium: $1 for each hour worked between 4:30 pm in the warehouse and 10:30 p.m. in the garage, and until the start of the day shift. $1 for each hour worked in the freezer on days or nights. Paid holidays: 10 days. Vacations with pay: 1 day for each full month of employment under 1 year, to a maximum of 10 days. 2 weeks after 1 year, 3 weeks after 5 years, 4 weeks after 10 years, 5 weeks after 18 years (As of Jan. 1, 2017: 6 weeks after 30 years). Overtime: For a 5-day work week employee, time and one- half for all hours worked in excess of 8 hours in a day and/or 40 hours in a week, and for all hours worked on the sixth and seventh day of the employee's regular work week. For a 4-day work week employee, time and one-half for all hours worked in excess of 10 hours in a day and/ or 40 hours in a week, and for all hours worked on the fifth, sixth and seventh day of the em- ployee's regular work week. Meal allowance: For drivers required to sleep away, break- fast $10, lunch $15, dinner $20. Drivers required to stay overnight while on route shall receive an overnight bonus of $50. Medical benefits: 100% of the premiums in the group in- surance plan. Supplementary Medical Expense Benefit 100% of eligible expenses subject to a $1 per prescription deductible. Dental: A level two or reason- able equivalent dental plan will be provided at a 2-year lag in current ODA (Ontario Dental Association) rates. Effective in the first full pay period fol- lowing March 19, 2020: Orth- odontic coverage for dependant children 50% reimbursement capped at a $1,500 lifetime maximum; major dental cover- age at 50% reimbursement to a lifetime maximum of $1,500; crowns at 50% reimburse- ment to a lifetime maximum of $1,500. Vision: Up to $225 every 24 months. Weekly indemnity: Sickness - Accident Weekly Indemnity Plan equal to 67% of employee's gross weekly earnings, cal- culated as the average gross weekly earnings of the prior 16 weeks, to a maximum benefit of $1,500 per week, for a period of 26 weeks. Sick leave: 40 paid sick hours per calendar year. LTD: 100% of the premium. Life insurance: 100% of the cost of the premiums in the present group insurance plan or its equivalent. Pension: Employer contrib- utes to SYSCO Pension Plan. Monthly contribution effective first full pay period following: March 19, 2016: $255; March 19, 2017: $260; March 19, 2018: $265; March 19, 2019: $270; March 19, 2020: $275. Bereavement leave: Up to 5 days for death of spouse, child, stepchild. Up to 3 days for father, stepfather, mother, step- mother, brother, stepbrother, sister, stepsister, father-in law, mother in-law. 1 day for grand- parents, brother-in law, sister- in law. Seniority – recall rights: 12 months Probationary period: 1,040 hours worked. Severance: 120 hours per year of service, pro-rated on a monthly basis (regularly hourly rate), to a maximum of 40 weeks. Safety shoes: $275 for seniority employees. Uniforms/clothing: Cold environment clothing for em- ployees who regularly work in the freezer including 1 freezer jacket, 1 pair of freezer pants, gloves as needed, 1 balaclava. Cooler jackets every 18 months for those employees who regu- larly work in the cooler. Tool allowance: Up to $625 per year for replacement or up- grading of tools, for mechanics. Up to $325 for mechanics and maintenance to purchase pre- scription safety glasses.
