Overtime: Time and one-half for all hours worked after regu- lar hours or 40 hours per week. Medical benefits: Single, de- pendant coverage (more than 9 years' service): employer pays 100% of premiums. Dependant coverage (less than 9 years' service): employer pays 80%, employee pays 20%. Dental: $2,000 maximum per year for preventative, basic cov- erage, major restorative. Vision: Year 1: $275; year 2: $275; year 3: $300 (previously $250 every 2 years). Paramedical: $600 per year for podiatrist, chiropodist, orthopedic. Maximum $1,250 per year combined coverage. Effective June 1, 2018: $1,350 per year. STD: 75% of salary, maximum 17 weeks. LTD: 60% of salary, from 18 to 26 weeks. AD&D: 2 times annual salary coverage. Life insurance: 2 times an- nual salary coverage, minimum $75,000, maximum $120,000. Pension: Employer will con- tribute 2.5% per year and will match employee contributions, maximum 1.5% per year. Bereavement leave: 5 days for death in immediate family (par- ent, guardian, spouse, same-sex spouse, partner, child). 3 days for death in extended family (brother, sister, father-in-law, mother-in-law, grandparents). 1 day for death of brother-in-law, sister-in-law. Seniority – recall rights: 24 months. Probationary period: 90 days (previously 60 days). Severance: 1 week's pay per year of service, after employee has worked 5 years. Safety shoes: $150 per year (previously $125 per year). Uniforms/clothing: Employer will supply uniforms, valet, laundry service. Tool allowance: $125 per year for knives. Employee must have: French knife, boning knife, par- ing knife, slicer/carving knife, peeler, steel, tongs, palette knife, piping bags and tips, tool box or secure wrap set, meat fork, temperature gauge, slotted spoons, ladles. Wage adjustments: Effective Jan. 1, 2016: 2% Effective Jan. 1, 2017: 2% Effective Jan. 1, 2018: 2% Shift premium: $2 per hour for actual hours worked between 6 p.m. and 6 a.m. Paid holidays: 12 days. Vacations with pay: 1 ¼ days per calendar month to start; 1 2/3 days per calendar month after 5 years; 2 1/12 days per calendar month after 10 years; 2 ½ days per calendar month after 20 years. 4 days' vaca- tion travel allowance once per year if at least 5 days are taken consecutively. $1,100 vacation travel allowance annually for permanent employees with at least 2 years' of service. $150 for each calendar month with 10 days worked. Overtime: Time and one- half for hours worked beyond regular daily and weekly hours. Double time for all overtime hours in excess of 4 in one day or 8 on the first day of rest. Double time for all hours worked on second and subse- quent days of rest. Meal allowance: Employer supplies a meal for employees required to work 3 hours or more of overtime immediately before or following their nor- mal hours of work. Medical benefits: 50/50 cost shared on premiums for cover- age. Dental: 50/50 cost shared on premiums for coverage. Vision: 50/50 cost shared on premiums for coverage. Paramedical: 50/50 cost shared on premiums for cover- age. Sick leave: 1 ¼ days per calen- dar month with at least 10 paid days of work. Up to 15 days can be advanced before they are earned. LTD: 50/50 cost shared on pre- miums. AD&D: 50/50 cost shared on premiums for 3 times annual salary maximum benefit. Life insurance: 50/50 cost shared on premiums for 3 times annual salary. Pension: All eligible employees must participate in Northern Employees' Benefits Services Pension Plan. Bereavement leave: 8 days for death of immediate family member. 2 days for sibling-in- law or child-in-law, with up to 3 additional trays for travel. Seniority — recall rights: 18 months. Probationary period: 6 months. Severance: 2 weeks' pay for first complete year of service and 1 week's pay for each ad- ditional full or partial year of continuous employment. Safety shoes: $200 annually for employees deemed to require safety footwear. Uniforms/clothing: Employer provides protective clothing where needed. $250 annually for coveralls, smocks and swim- suits deemed appropriate by department head.
