Employer will pay employee's regular rate of pay for attendance and travel time. Medical documentation: When employer requests employee provide a completed Attending Physician's Report (APR), employer will reimburse employee, maximum $60. Benefit supplement: All employees will receive $100 after ratification to supplement benefit plan. Effective June 2017: $100. Effective June 2018: $100. HOSPITALITY Radisson President Hotel & Suites Vancouver (90 hotel staff ) and Unifor, Local 3000 Renewal agreement: Effective June 1, 2016, to May 31, 2019. Signed on Oct. 19, 2016. Wage adjustments: Effective June 1, 2016: 2% Effective June 1, 2017: 2% Effective June 1, 2018: 2% Shift premium: $0.75 for bell- service employee working shift that begins between 10 p.m. and midnight. $1.50 per hour when employer assigns employee to work as lead server. Paid holidays: 11 days. Vacations with pay: 2 weeks or 4% after 1 year, 3 weeks or 6% after 3 years, 4 weeks or 8% af- ter 7 years, 5 weeks or 10% after 18 years. Overtime: Double time for all work after 8 hours per day, 40 hours per 5-day period, 5 days per 7-day period. Meal allowance: 1 meal per day will be provided for all shifts longer than 5 hours. Medical benefits: Employer pays 100% of premiums after employee completes 6 consecu- tive pay periods averaging 20 hours per week. Vision: $300 every 24 months. AD&D: Coverage of $30,000. Life insurance: Coverage of $30,000. Bereavement leave: 3 days for death in immediate family (mother, father, legal guardian, son, daughter, sister, brother, spouse, partner, current father- in-law, current mother-in-law, current brother-in-law, current sister-in-law, grandchildren, grandparents, stepparents, stepchildren, same-sex partners and children as long as they live with employee). Seniority – recall rights: 6 months. Probationary period: 60 work shifts, maximum 4 months. Discipline: Sunset clause is 12 months. Severance: 12 hours' pay per year of service. Safety shoes: Employer will reimburse 50% of cost of safety footwear, maximum $75 per life of agreement. Uniforms/clothing: If any special uniform is required, such as tuxedo or white jackets, employer will supply same and be responsible for cleaning. All uniforms or special articles of wearing apparel worn by employee while on duty will be supplied and laundered by employer. (270 health-care workers, cooks) and the Health Care and Service Workers Union (CLAC), Local 304 Renewal agreement: Effective June 1, 2016, to May 31, 2019. Signed on April 3, 2017. Shift premium: $1 per hour for training new HCA/PSW and dietary employees and trainer is not designated as extra. $1 per hour for employee designated as trainers in housekeeping/ laundry, maintenance and recreation departments. $0.40 per hour for work on evening or night shift. $0.30 per hour (previously $0.25 per hour) for hours worked between 11 p.m. Friday and 11 p.m. Sunday. Effective Jan. 1, 2018: $0.35 per hour. $1.25 per hour to employees whose workload is increased when another employee who is scheduled does not report for work and is not replaced. $10 per shift for RPN who supervises 2 floors due to lack of registered staff. $1 per hour when employer temporarily assigns employee to carry out responsibilities of department head. $10 per shift "in charge" premium when reg- istered practical nurse assumes responsibilities of unit supervi- sor or shift supervisor. $0.20 per hour for employee who is quali- fied as physiotherapy aid or occupational therapy aide and
