Maxi- mum $120 every 24 month- period for 1 eye examination for employee, spouse. Weekly indemnity: 66 2/3% of base rate times 40 hours, maxi- mum $1,050 (based on 1-4-26 formula). $1 premium per hour for each hour worked on shift that begins after noon.
