Medical benefits: Employer pays 100% of premiums Dental: Employer pays 100% of premiums for 50% reim- bursement for orthodontics for children, lifetime maximum of $2,000. Maximum $2,000 per year for basic coverage. Vision: Employer pays 100% of premiums for coverage of $275 every 24 months (previously $230 every 24 months). Em- ployees, families will be eligible for 1 eye exam every 24 months, maximum $85 per exam. Sick leave: 1 day after 3 months of service; 3 days after 6 months of service; 6 days after 1 year of service; 10 days after 2 years of service; 12 days after 3 years of service; 14 days after 4 years of service. Employer will pay employee time and one-half for each day of unused sick leave in February or March of each year. STD: Employer pays 100% of premiums for 2/3 of weekly earnings, maximum 15 weeks. LTD: Employer pays 100% of premiums for 66 2/3% of weekly earnings, maximum $2,500 per month. AD&D: Employer pays 100% of premiums for 1 times annual earnings. For employees hired after Feb. 6, 2011, DC plan in which em- ployee contributes minimum 2%, employer contributes 3% and initial deposit of $2,200 (4% after 5 years of service, 5% after 10 years of service). Bereavement leave: 5 days for death of spouse, child, parent. 3 days for death of sister, brother, mother-in-law, father-in-law, sister-in-law, brother-in-law, daughter-in-law, son-in-law, stepparents, stepchildren, grandparents, grandparents of spouse, grandchildren, grand- children of spouse. Seniority – recall rights: 24 months with less than 5 years of seniority, 36 months with more than 5 years of seniority. Probationary period: 90 days during 6-month period. Severance: If North Bay plant closes permanently, employer will increase maximum under severance pay regulations from 26 weeks to 36 weeks for all full- time permanent employees. Safety shoes: $210 per year (previously $190 per year) ef- fective May 1 of each year. If employee prefers allowance, it will be $150 per year, which is taxable. Shift premium: $2.90 per hour when employee is assuming additional responsibilities of coker push. $5.50 per hour (pre- viously $5 per hour) when per- forming duties of maintenance supervisor, maintenance co- ordinator, operations coordin- ator, shift supervisor, laboratory supervisor, block supervisor for major shutdowns. $1,500 per year for first-class, second-class and $600 per year for third- class for utilities technicians who possess valid permanent third-, second- or first-class steam ticket. Paid holidays: 12 days. Vacations with pay: 3 weeks after 1 year, 4 weeks after 10 years, 5 weeks after 18 years, 6 weeks after 25 years. Overtime: Paid for all work af- ter daily scheduled hours. Meal allowance: When em- ployee is required on regular day of work, to stay over and work 2 or more hours of over- time, employer will supply employee with meal after first 2 hours of work and every 4 hours thereafter. When employee is scheduled to work overtime on normal day of rest or called in on overtime, employer will sup- ply employee with meal after 4 hours of work and every 4 hours thereafter.
