Shift premium: $2.10 per hour (previously $1.95 per hour) for each hour worked which falls within normal hours of evening shift. $2.35 per hour (previously $2.20 per hour) for each hour worked which falls within nor- mal hours of night shift. $2.45 per hour (previously $2.30 per hour) per hour for each hour worked between midnight Friday to midnight Sunday or such other 48-hour period that employer may establish. $0.60 per hour when employee is as- signed additional responsibility of providing preceptorship to students or mentorship to other employees. Paid holidays: 11 days, plus 1 float day. Vacations with pay: 3 weeks to start, 4 weeks after 3 years, 5 weeks after 12 years, 6 weeks after 21 years, 7 weeks after 27 years. Overtime: Time and one-half for all work after 7 1/2 or 8 hours per day or 35 or 37 1/2 hours per week. May be taken as time off, within 4 months, or paid out, maximum 5 days. Meal allowance: Meal vouch- er, maximum $6 value, or $6 if employer is unable to provide meal voucher for employee who works more than 2 hours of overtime immediately following scheduled hours of work. Medical benefits: Employer pays 80% of premiums, employ- ee pays 20% of premiums. Dental: Employer pays 80% of premiums, employee pays 20% of premiums. Sick leave: 3/4 of day per month, maximum 9 days per year. May be accumulated, maximum 27 days. STD: 70% of weekly earnings beginning at latest on 10th day of absence, maximum 26 weeks. LTD: Employer pays 80% of premiums, employee pays 20% of premiums for coverage pay- able after 26 weeks of disability at 66 2/3% of earnings. Pension: Hospitals of Ontario Pension Plan. Bereavement leave: 5 days for death of spouse, mother, father, stepparent, son, daugh- ter, stepchild, brother, sister. 3 days for death of mother-in- law, father-in-law, son-in-law, daughter-in-law, sister-in-law, brother-in-law, grandparent, grandparent-in-law, grandchild, guardian, ward, aunt, uncle, niece, nephew. Seniority – recall rights: 24 months. Call-in pay: Minimum 2 hours' pay at time and one-half. Probationary period: 60 tours worked from date of last hire (450 hours of work for em- ployees whose regular hours of work are other than standard workday). Discipline: Sunset clause is 18 months. Any absences of 30 consecutive days or longer will not be counted in calculating 18-month period. Safety shoes: $125 per year for CSA-approved safety footwear. Uniforms/clothing: Employer will provide, where required, appropriate protective clothing.
