Maximum $1,000 per year for basic services. Life- time maximum $6,000 for re- storative, orthodontic services. Vision: Employer pays 100% of premiums. $250 every 24 months for frames, lenses, fitting of prescription glasses and contact lenses for each insured person and for depend- ent children younger than 18. Maximum $1,000 lifetime for laser eye surgery for employees only. Maximum $60 per person every 24 months for eye exam- inations. Paramedical: Employer pays 100% of premiums. $500 per year for physiotherapist. $350 per year for chiropractor, podi- atrist, speech-language pathol- ogist, psychologist. Maximum $1,200 reimbursement per year for all practitioners. Weekly indemnity: 75% of wages during 26-week period. Sick leave: 6 sick-day credits per year. Cannot be carried over year-to-year. LTD: 60% of wages. AD&D: Coverage of $83,000 (previously $81,500). Life insurance: Employer pays 100% of premiums for coverage of $83,000 (previously $81,500). Pension: DB pension plan for all full-time employees hired before March 23, 2010. Employ- er contributes 6%, mandatory employee contribution of 6% for DC pension plan for all full-time employees hired after March 23, 2010. Bereavement leave: 3 days for death in immediate family (wife, husband, daughter, son, mother, father, sister, brother, mother-in-law, father-in-law, grandparents (of employee or wife), brother-in-law, sister-in- law, nieces, nephews, grand- children) if funeral is in Alberta; 4 days if outside Alberta; 5 days if service is outside Canada. 1 day for death of aunts, uncles. Seniority – recall rights: 24 months. Probationary period: 60 work- ing days. Discipline: Sunset clause is 1 year for verbal warnings, 2 years for disciplinary records.
