Time and one-half for next shift when employer fails to provide full-time employee cancellation notice no less than 24 hours prior to commencement of shift or part-time employee 12 hours prior to the commencement of shift. $1.25 per hour when employee works as unit leader to direct or oversee work of employees within their clas- sification or be assigned overall responsibility for patient care on unit, ward or area for 1 tour. $1.25 per hour when senior storekeeper operates vehicle as part of assignment and vehicle requires DZ Ontario driver's licence to operate. Paid holidays: 12 days. Vacations with pay: 1.25 days per month to start, 3 weeks after 1 year, 4 weeks after 5 years, 5 weeks after 13 years, 6 weeks after 22 years, 7 weeks after 28 years. Overtime: Time and one-half for all work after 7.5 or 8 hours per day or 37.5 or 40 hours per week. May be taken as time off in lieu. Meal allowance: Maximum $7 for employee who works more than 2 hours of overtime immediately following sched- uled hours of work without notification. Medical benefits: Employer pays 75% of premiums, em- ployee pays 25% of premiums. Employer pays 100% of pre- mium for Hospitals of On- tario Group Life Insurance Plan (HOOGLIP). Dental: Employer pays 75% of premiums, employee pays 25% of premiums. $1,500 maxi- mum lifetime for orthodontics. $1,500 maximum per year for each of crowns and dentures on 50/50 co-insurance basis. Vision: Employer pays 75% of premiums, employee pays 25% of premiums. Coverage of $300 every 24 months (previ- ously $250 every 24 months). Effective April 1, 2020: $350 every 24 months. $100 every 24 months for eye examinations. Paramedical: Employer pays 75% of premiums, employee pays 25% of premiums. Com- bined maximum of $300 per year for naturopath, chiroprac- tor, homeopath, podiatrists, massage therapist. LTD: Employer pays 75% of premiums, employee pays 25% of premiums for HOODIP or equivalent plan. Pension: Employees will enroll in Hospitals of Ontario Pension Plan (HOOPP). Bereavement leave: 5 days for death in immediate family (spouse, common-law spouse, partner of same sex, child, step- child, parents, stepparents). 3 days for death of other mem- bers of family (father-in-law, mother-in-law, brother, sister, brother-in-law, sister-in-law, grandparents, spouse's grand- parent, grandchild, son-in-law and daughter-in-law). 1 day to attend funeral of or memorial service (or equivalent) for aunt, uncle. Seniority – recall rights: 30 months; 18 months for em- ployee transferred to position outside of bargaining unit for more than 3 months. Call-in pay: Minimum 4 hours' pay at time and one-half regular hourly rate. May be taken as time off in lieu with 60 days. Probationary period: 450 hours or first 12 months. Discipline: Sunset clause is 18 months. Severance: 2 weeks' salary for each year of service, maximum 26 weeks' pay, plus, within 12 months of resignation, em- ployees may be reimbursed for tuition fees, maximum $3,000. 4 weeks' salary plus, within 12 months of resignation, em- ployees may be reimbursed for tuition fees, maximum $1,250. Safety shoes: $0.05 per hour for safety shoes that do not require green triangle. $0.07 per hour for safety shoes that require green triangle. Uniforms/clothing: $0.06 per hour for uniforms to all employees who wear uniforms while on duty which employer does not supply. Mileage: $0.40 per kilometre if employee uses own automobile on employer's business.
