Vision: Coverage of 1 pair of regular lenses, 1 set of frames (maximum $25), 1 set of contact lenses (maximum $40), prescribed contact lenses for certain corneal disabilities where regular glasses are unsuitable (maximum $200). Paramedical: Maximum $150 per person (maximum $300 per family) per year for chiropractors, naturopaths. Weekly indemnity: Coverage on first day of accident, eighth day of sickness, $462 per week (maximum 85% of net earnings) for 52 weeks. LTD: Coverage of $1,000 per month, after 52 weeks waiting period. AD&D: Coverage of $25,000. Life insurance: Coverage of $50,000. Bereavement leave: 3 days for death in immediate family (including in-laws). Sample rates of per-load pay (current): Owner-operators: 57% of rev- enue generated by load; 90% of time for waiting time; 55% on suspended orders; 75% of monies collected for in-transit moves. Medical examinations: Employer will pay for all examinations for owner-operators with more than 2 years' seniority. If employer elects to exercise its right to select its own medical examiner or physician for medical examination of owner-operator with less than 2 years' seniority, employer will pay for same. Weekend-availability premium: $2,200 per year to owner-operators who make themselves available for weekend dispatches and when called, accept such dispatches. Owner-operator must be available minimum 26 weekends within each 12-month period to qualify and be eligible for payment (12:01 a.m. Saturday through to midnight Sunday).
