Vacations with pay: 0.77 days per bi-weekly period of em- ployment to start, 0.96 days per bi-weekly period of em- ployment after 12 years, 1.15 days per bi-weekly period of employment after 19 years and 1.34 days per bi-weekly period of employment after 28 years. Employees will not accumulate vacation credits in excess of 30 days. Overtime: Time and one-half for all hours worked beyond 7 hours per day of 35 hours per week. Medical benefits: $500 every 36 months for hearing aids. Dental: $2,500 per person per year for basic and major cover- age. $1,750 lifetime maximum for orthodontic work. Vision: $350 every 2 years for corrective frames or lens- es. $54 every 2 years for eye exams. Sick leave: 1 week to start, 2 weeks after 1 year, 3 weeks after 2 years, 4 weeks after 3 years, 5 weeks after 4 years, 7 weeks after 5 years, 9 weeks after 6 years, 11 weeks after 7 years, 13 weeks after 8 years and 17 weeks after 9 years. LTD: Coverage starts on the 180th day of disability. Paid at the 66.67% of employee's regular rate of pay. Maximum $4,500 per month. AD&D: Employer pays 100% of premiums. Double annual earnings to a maximum of $200,000. Terminates at age 70 or retirement. Life insurance: Employer pays 100% of premiums. Double annual earnings to a maximum of $200,000. Bereavement leave: 7 days for employee's spouse or child. 5 days for employee's parent, brother, sister, grandparent, grandchild, spouse's parent, spouse's grandparent, broth- er-in-law, sister-in-law, son- in-law or daughter-in-law. 1 day for employee's uncle, aunt, nephew or niece. Seniority – recall rights: 2 years. Call-in pay: Minimum 4 hours at time and one-half. Probationary period: 4 months or 600 hours worked, whichever comes first. Discipline: Sunset clause is 1 year. Personal leave: Requests for unpaid personal leave may be granted provided the employee provides reasonable notice, in writing. Family medical leave: Up to 8 weeks within a 26-week period. Maternity leave: 17 weeks. Parental leave: 35 weeks or 37 weeks when pregnancy leave is not taken. Military leave: Unpaid military leave will be granted in accordance with the Employment Standards Act. Standby: $2.90 per hour. $4.35 per hour on paid holidays. Split shifts: There will be no split shifts. HEALTH CARE Southwest Community Care Access Centre Southwest Ontario (390 nurses and support services employees) and the Ontario Nurses' Association Renewal agreement: Effect- ive April 1, 2014, to March 31, 2016. Signed on May 13, 2015. Wage adjustments: Effective April 1, 2014: 1.4% Effective April 1, 2015: 1.4% Shift premium: $1.85 per hour worked after 4:30 p.m. $2.20 per hour worked on the weekend. Paid holidays: 11 days. Vacations with pay: 1.67 days per month worked to a max- imum of 20 days to start, 2.08 days per month worked to a maximum of 25 days after 10 years and 2.5 days per month worked to a maximum of 30 days after 17 years. Overtime: Time and one-half for all hours worked beyond 70 hours per 2-week period. Meal allowance: $13 after 2 hours of overtime. Medical benefits: Employer pays 100% of premiums. $8 cap on dispensing fee for pre- scriptions. $300 per year for services from acupuncturist, naturopath, osteopath or podiatrist. $400 per year for services from physiotherapist, chiropractor and registered massage therapist. $300 every 36 months for hearing aid plan. Vision: $350 every 2 years. LTD: Employer pays 80% of the premium cost. 66% of monthly earnings to a max- imum of $4,000 per month. Bereavement leave: 5 days for employee's spouse, partner, child, parent, brother, sister, stepparent or stepchild. 3 days for employee's grandparent, grandchild, brother-in-law, sister-in-law, father-in-law, mother-in-law or legal guard- ian. 1 day for employee's uncle, aunt, cousin, nephew or niece. One-half day to serve as a pall- bearer. Seniority – recall rights: 2 years. Call-in pay: Minimum 4 hours' pay at time and one- half. Mileage: 47¢ per km. Standby: $3.50 per hour. $4.50 per hour on paid holi- days. SERVICES Brink's Canada Provincewide, Alberta (220 armoured car services employees) and the Teamsters Renewal agreement: Effect- ive Sept. 20, 2014, to Sept. 19, 2018. Wage adjustments: Effective Sept. 20, 2014: 2.4% Effective Sept. 20, 2015: 2% Effective Sept. 20, 2016: 1.6% Effective Sept. 20, 2017: 3.6% Shift premium: $1 per hour for employees who work 4 consecutive 10-hour shifts in a split-days-off work schedule.
