Vision: Younger than 21: $300 per year. 21 and older: $300 every 2 years. STD: Coverage of 60% weekly basic earnings, maximum $600 per week. Payable after first day of accident or hospitalization or 14th day of illness, maximum 119 days. LTD: Coverage of 60% week- ly basic earnings, maximum $2,600 per month. Payable after 119 days.
