Shift premium: $0.28 for each hour worked on afternoon or evening shifts only for employ- ees who rotate over 2 or more shifts. $0.30 per hour (previ- ously $0.15 per hour) for work between 11 p.m. Friday and ending at 11 p.m. Sunday. Paid holidays: 10 days, plus 2 float holidays. Vacations with pay: 4% to start, 2 weeks or 4% after 1 years, 3 weeks or 6% after 4 years, 4 weeks or 8% after 8 years, 5 weeks or 10% after 15 years, 6 weeks or 12% after 23 years, 7 weeks or 14% after 28 years. Overtime: Time and one-half for all work after 7 1/2 hours per day or 75 hours in 2-week period. May be taken as time off in lieu. Medical benefits: Employer pays 100% of premiums for Blue Cross Extended Health Care Plan. Dental: Employer pays 50% of premiums, employee pays 50% of premiums. Vision: Employer pays 100% of premiums for $250 every 24 months (previously $160 every 24 months). Sick leave: 1.25 days per 162.5 hours worked, maximum ac- cumulation 45 days, after 3 months of service. Life insurance: Employer pays 100% of premiums for coverage of $30,000. Pension: Employer contributes 4%, employee contributes 4% to Teamsters Pension Plan. Bereavement leave: 4 days for death of spouse, child, step- child, parent, sibling. 3 days for death of mother-in-law, father-in-law, brother-in-law, sister-in- law, guardian, grand- parent, grandchildren, son-in- law, daughter-in-law. 2 days if funeral is not attended. 1 day for death of aunt, uncle, niece, nephew. Seniority – recall rights: 36 months. Probationary period: 450 working hours. Discipline: Sunset clause is 12 months, except in case of incidents involving third-party interface such as residents and family where records will remain on file; 18 months for suspensions.
