Dental: Dental plan partially covered by employer. Vision: $350 per 24-month pe- riod for lenses, frames, contact lenses, laser eye surgery. STD: Employer pays 100% of premiums for coverage up to 26 weeks. LTD: Employer pays 100% of premiums for employees with at least 2 years of seniority. AD&D: Employer pays 100% of premiums for group plan. Life insurance: Employer pays 100% of premiums for group plan.
