Overtime: Double time for all work after 7.75 hours per day or on scheduled days of rest. May be banked and taken as time off in lieu. Time off not taken by last day of March in any given year will be paid out. Medical benefits: Employer pays 60% of premiums, em- ployee pays 40% of premiums. Dental: Employer pays 60% of premiums, employee pays 40% of premiums. Coverage of $2,000 maximum per year. Preventative at 80% cover- age (check-ups, cleanings, X-rays); minor restorative at 80% (fillings, extractions); endo, perio at 80% (root ca- nals, gum disease); major restorative at 50% (crowns, caps, bridges, dentures); or- thodontics at 50% coverage (braces) up to 19 maximum $3,000 per lifetime. Vision: Employer pays 60% of premiums, employee pays 40% of premiums. Coverage for eyeglasses, lenses, frames, contacts, laser eye surgery, maximum $300 every 24 months for adults, maximum $300 every 12 months for children. Eye exams, maxi- mum $75 every 24 months for adults, maximum $75 every 12 months for children Paramedical: Coverage of 80% for physiotherapy, chi- ropractor, psychologist, mas- seur, acupuncture, podiatrist, speech therapist, naturopath, osteopath, chiropodist, so- cial worker, maximum $750 per year per practitioner per person. Sick leave: 1.25 days for each month of employment, maxi- mum 15 days per year, after probationary period ends. Employee may carry forward unused portion of leave for 1 year, maximum 30 days. Em- ployee who does not use any paid sick leave days for 1 year will be given 1 paid day off to be taken in following year. STD: Employee pays 100% of premiums for coverage of 67% of earnings, maximum $1,000 per week. LTD: Employee pays 100% of premiums for coverage of 66.67% of first $3,000 of monthly earnings, plus 55% of next, after 17 weeks. AD&D: Employee pays 100% of premiums. Benefit paid is double if death due to ac- cident. Life insurance: Employee pays 100% of premiums for coverage of 1 times annual earnings. Pension: Employee may con- tribute 3% per hour worked (matched by employer, maximum 3%) into employer administered, employee self- directed, RRSP. Bereavement leave: 3 days for death in immediate fam- ily (spouse, child, parent, brother, sister, niece, nephew, aunt, uncle, mother-in-law, father-in-law, brother, sister, brother-in-law, sister-in-law, son-in-law, daughter-in-law, grandparent, grandchild, guardian or fiancé). Spouse will include common-law, same-sex relationship. Step- parent, stepchildren, step- brother, stepsister will be considered as members of employee's immediate fam- ily. 2 additional days if more than 500 kilometres of travel is required. Seniority – recall rights: 12 months for layoffs. Probationary period: 450 hours worked, exclusive of overtime hours worked; for part-time, casual employees who have 6 calendar months of service but who have not completed 450 hours, pro- bationary period, will be deemed to have been com- pleted. May be extended ad- ditional 450 hours, maximum 900 hours overall. Discipline: Sunset clause is 2 years for employee who has been subject to disciplinary action. Severance: 1 week's pay after 3 months of service; 2 weeks' pay after 2 years of service; 4 weeks' pay after 4 years of ser- vice; 5 weeks' pay after 6 years of service; 3 weeks' pay after 8 years of service; 8 weeks' pay after 10 years of service.
