Double time and one-half for hours worked on statutory holidays in excess of 8 on day. May be taken as compensatory time but must be taken within 4 weeks of time worked. Medical benefits: Employer pays 100% of premiums for ex- tended health care plan. Dental: Employer pays 100% of premiums for plan equal to 2002 O.D.A. rates. Vision: Employer pays 100% of premiums for coverage of $300 per 24 months per employee's family member and 1 eye exam every 24 months not covered by OHIP. Sick leave: 1 day per month, maximum of 90 days. Up to 5 days per year can be used for at- tending medical appointments or care for ill family members. Can be used to supplement STD or LTD claims. STD: Employer pays 100% of premiums for coverage of 66 2/3% of weekly earnings for up to 17 weeks with employer covering difference between in- surer's maximum and 66 2/3% level. LTD: Employer pays 100% of premiums for coverage of 66 2/3% of monthly earnings up to $2,500 per month. AD&D: Employer pays 100% of premiums for coverage of double employee salary to near- est $1,000. Life insurance: Employer pays 100% of premiums for coverage of double employee salary to nearest $1,000. Pension: Employer contributes 5.5%, employee contributes 5% into Multi-Sector Pension Plan (MSPP). Bereavement leave: 5 paid days for funeral or memorial of parent, spouse, child. 3 paid days for funeral or memorial of sibling, grandparent, grand- child, parent-in-law, spouse's grandparent, sibling-in-law. 1 paid day for funeral or me- morial for aunt, uncle, niece, nephew. One-half day without pay for attending funeral as pall bearer or mourner. Seniority – recall rights: 12 months after layoff; for em- ployee who is transferred to temporary position outside bar- gaining unit. Call-in pay: Minimum 3 hours' pay when called to work out- side regular scheduled working hours at overtime rate. Probationary period: 12 months. Discipline: Sunset clause is 24 months; 12 months for medica- tion errors. Severance: No less than 8 weeks' notice of layoff. Safety shoes: $200 per year for employees who are required to purchase safety boots for per- formance of duties. Uniforms/clothing: $75 per year bathing suit allowance for staff who regularly go swim- ming in municipal pool with people they support. Mileage: $0.46 per kilometre travelled in personal vehicle in performance of job duties for employer. Employer will pro- vide $100 reimbursement per year upon proven additional costs for business use insurance business rider for staff who use personal vehicles to perform jobs and in previous fiscal year averaged more than 300 kilo- metres per month.
